Position Summary: Reporting to the Senior Director of Home Health Operations, the Care Continuum Specialist serves as an internal resource and is responsible for promoting and coordinating additional services to Home Health patients. The Care Continuum Specialist will achieve this objective by proactively collaborating with the Care Coordination Team, clinicians, patients and families and promoting HealthFlex as the provider of choice. The primary focus/responsibility of this role is to meet the ongoing care needs of our clients by introducing additional services when appropriate. Schedule: Monday-Friday, Full Time - Exempt Location: This is a remote/field position, must be willing to travel across the SF Bay Area Office: Home office is located in Oakland, CA Essential Duties and Responsibilities: This section describes the essential functions of this role; however, this is not intended to be all-inclusive. Proactively collaborates with members of the Care Coordination Team in order to identify patients that would benefit from palliative or hospice services, including daily review and tracking of the Strategic Healthcare Programs platform, chart review and other resources as applicable. Creates and maintains a tracking method identifying and following existing home health patients that are appropriate candidates for services. Provides resources and education to members of the Home Health clinical team on how to identify appropriate levels of care, best timing for service introduction and benefits of palliative and hospice services Meets with patients and families regarding the benefits of various services and supports the client as they move through the continuum of care process Works closely with the Care Coordination Team to ensure a smooth and efficient transition between services Establish and maintain strong working relationships with both the home health and hospice care teams, patients, and families. Maintains an in-depth understanding of home health and hospice services and makes continuous improvements to the continuum of care services in order to meet the needs of the patient population resulting in increased internal referrals. Ability to effectively identify and provide exceptional communication to potential clients about the things that make our agency extraordinary in the delivery of patient care. Ensures compliance with administrative, legal and regulatory requirements Increase in volume and timing of patients moving to the right level of care. Compensation: This position offers a base salary range of $90,000-$115,000 plus commission. Compensation is determined by the following factors; experience, knowledge, skills, location, as well as internal equity and alignment with market data.
