Care Team Clinical Leads (CTCL) Permanent Full-Time Reporting to the Client Care and Operations Manager, the CTCL is a clinical leadership position who provides clinical support, education, and training to PSWs within dedicated geographical teams. As a clinical lead, the CTCL will support high quality home support service to clients in the community setting that is compliant with VHA Home HealthCare policy, Ontario legislation and College of Nurses of Ontario (CNO) standards of best practice. The CTCL will promote a culture of interprofessional collaboration among the team and champion a holistic, client and family-centered approach to care. For every client admitted for personal support, you will conduct an assessment and jointly develop the Client Care Plan based on the client care needs and authorized service plus provide ongoing monitoring of the care plan and client safety. To ensure quality in-service provision, you have excellent assessment skills and understanding of population-based risk. The CTLS will be working closely with Home and Community Care Support Services and other members of the interprofessional team (PT and OT) as needed to ensure client and PSW safety at point of care. In this role, the CTCL will conduct and follow up on incident investigations as assigned by the Care Team Supervisor and Client Care & Operations Manager. The CTCL will participate in clinical focused committees and working groups, as well as liaise with Home and Community Care Support Services (HCCSS) Care Team.
