CCPN $25000 Grant for a 2-year Full-Time Commitment CarePartners is one of Ontario's largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics. We continuously innovate to meet patient needs with specialized (award-winning) care programs and technology, including virtual care and AI. The NSWOC will provide wound and ostomy consultation for patients in the clinic and community. If you enjoy caring for a broad range of clients with a variety of wound etiologies and wound care modalities this is the job for you. In addition to make full use of your wound and ostomy care knowledge, each day you will use your skills in promoting self-management by patients and leading the interdisciplinary team to achieve optimal patient outcomes. Your expertise in the management of wounds and ostomies will be sought out by individual practitioners and the various committees and organizations that guide wound care in the region. CarePartners will provide you with an in-depth orientation to both the agency and wound/ostomy care in the community; ongoing clinical support including online access to clinical expertise; We are looking for Nurses with NSWOC certification who desire great flexibility in their schedules to support patients in their home & community. Practice within your full nursing scope Continuing education, certification and training Competitive wage grid that recognizes nursing skillset Flexible hours, autonomy over your schedule and a variety of assignments Incredibly supportive, caring and inspiring culture Comprehensive orientation and hands-on mentoring from experienced, caring staff Up-to-date mobile devices with current scheduling and virtual technology Robust Employee Assistance Program (EAP) for you and your family members Health & dental benefits, plus retail discounts and recognition rewards Opportunities to volunteer in countries with limited access to healthcare Using your knowledge of assessment, prevention and interventions regarding wound infections and ostomy care Care for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIs critically appraising the wound care literature and apply research evidence to practice Providing edema management including bandaging based on complete lower leg assessment Monitoring criteria for adjunctive therapy for complex or recalcitrant wounds Supporting our patient's safety by practicing according to policy and standards Using your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Home and Community Care Support Services (HCCSS) referral and other sources Functioning as a member of a nursing team who are accountable for the nursing care it provides Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending Several years of escalating experience managing wounds and ostomies A valid driver's license and vehicle as regular travel is required Excellent English verbal and written communication skills Current immunization record including 2 step TB test CarePartners In Your Community: In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
