The Alberta Children's Hospital (ACH) is a warm & vibrant workplace, with team members providing service to children & their families in inpatient units & ambulatory clinics. The Social Worker III, ensures clinical practice is patient/family centered, delivered in accordance with the Social Work Standards of Practice and Code of Ethics, evidence based, research informed, and aligned with program/clinic frameworks. The Social Worker III provides advanced level clinical assessment & intervention to patients and families and provides a range of advanced practice psychosocial supports & supportive counselling within assigned clinical service areas. Within the context of family centered care, the Social Worker III provides collaborative /shared leadership in the areas of team functioning and clinical consultation within the inter-professional health care team, including a range of service providers. The Social Worker III in this role provides primary psychosocial support to patients and families identified as having complex and chronic medical needs, including supporting referrals in areas such as CHIRP Clinic and Eating, Feeding and Swallowing Clinic amongst others. In addition to the advanced clinical skills required in this role, the Social Worker will provide psychosocial care coordination within ACH; may include other government ministries and community –based care providers. The Social Worker in this role will demonstrate advanced critical thinking and professional judgment, the ability to conceptualize and manage complex cases from a range of diverse perspectives in order to facilitate agreed-upon outcomes. The Social Worker will integrate & synthesize multiple levels of information in adherence with evidence-based practice, professional standards & applicable legislation, policy, and guidelines. As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s). Additional Required Qualifications: A minimum of 3-5 years of experience in pediatric social work, in an acute health care setting.Formal training and demonstrated ability to deliver trauma–informed interventions and crisis interventions.Formal training and demonstrated experience supporting families through grief and loss.Demonstrated experience with the intersection of child welfare and medical Social Work to address child maltreatment and neglect.Advanced interpersonal and communication skills and advanced clinical intervention including counselling or therapy for a range of complex and multidimensional patient and family needs.Demonstrated skills in formal counselling interventions.
