We are currently seeking the right person to join our team in the Health Services neighborhood as a Licensed Practical Nurse

As a Licensed Practical Nurse (LPN) with Touchmark, you will enrich lives every day by ensuring residents in our Health Services neighborhood receive the highest level of dignity and care. You will have the daily opportunity to distinguish yourself by taking the time to be a positive presence in the lives of residents and their family members. As the LPN, you will support our nursing team in coordinating all clinical care and ensuring the residents receive the highest quality of care. Working in a Health Services community provides you the opportunity to build meaningful relationships with residents and team members through daily interactions. This is a part-time position in day shift.

The ideal candidate will be trustworthy and compassionate, and will:

  • Have a genuine interest in caring for aging residents and working in a geriatric environment.
  • Possess a current LPN license.
  • Enjoy being part of a team, as this person will work closely with a team of nurses to design and implement care plans for each resident.
  • Ability to read and write English in order to follow and give oral and written instructions.

Responsibilities:

  • Help oversee, mentor and coach the care staff and assist them in making educated care decisions.
  • Provide hands-on nursing care for residents residing in Health Services Neighborhood
  • Assess the health status of residents, as needed, and work with the nursing team and health services team to develop and review individualized service plans.
  • Develop trust and strong communication with family members to be able to provide information and insight related to the resident's health and well-being.
  • Must be able to complete required paperwork and filing with a high degree of accuracy.
  • Ability to observe resident's behavior and to communicate observations in a professional and intelligent manner.
  • Physically capable of turning, stooping, bending, stretching, and moving equipment and performing other related tasks.
  • Be able to assess nursing care given by yourself and non- professional staff members. be able to identify problems and or need for improvement.
  • Be able to collect data from resident, family and significant others, health care providers, individuals, and or agencies in the community.
  • Assist with the orientation of new employees.
