Overview

The Connected Care Halton Ontario Health Team (CCHOHT) was one of the first Ontario Health Teams to be approved by the Ministry of Health and continues to work towards a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.

This partnership has mapped out an ambitious and challenging strategy to improve the experience patients will have when accessing care. While the CCHOHT's approaches are grounded in population health approaches, these innovative strategies include early intervention, new digital technologies, enhanced bundled care programs, and increased coordination for medically complex patients. True to its population health roots, the CCHOHT is also especially focused on the social determinants of health, and developing approaches for marginalized and disadvantaged populations who experience higher rates of health issues.

The CCHOHT represents a partnership of health care providers in the Halton Hills, Milton and Oakville communities committed to better population health, better patient experiences, reduced costs and a better care team experience. The CCHOHT is guided by a Collaborative Committee with equal leadership representation from Acclaim Health, Mississauga Halton Home and Community Care Support Services, Halton Healthcare, Halton Region as well as physician representations (primary care from within the geographical areas) and the chair of our Patient, Family and Caregiver Advisory Committee.

Key Accountabilities

Reporting to the Collaborative Committee, the Executive Director CCHOHT is responsible for operationalizing and implementing the advancement and success of the CCHOHT by leading all aspects of the strategy, planning, development, implementation and growth of the CCHOHT. The Executive Director will:

  • Provide leadership to the development and ongoing implementation of the CCHOHT working with the Ontario Health guidelines and the Collaborative Committee to provide support, including supporting strategy development and decision-making required for implementation of the CCHOHT Strategic Plan and Annual Operating Plan.
  • Coordinate the Primary Care Network (PCN) with the goal of connecting, integrating and supporting primary care providers within the CCHOHT, including but not limited to the management of chronic disease.
  • Liaise with Ontario Health and the Ministry of Health about the ongoing development of the CCHOHT, with a focus on advancement aligned with Ontario Health and Ministry of Health priorities for high performing Ontario Health Teams.
  • Convene a wide range of partners from the full continuum of health care and social service settings to advance the Strategic Plan and Annual Operating Plan of the CCHOHT.
  • Develop effective and collaborative working relationships with CCHOHT membership and broader health and social service providers, other OHTs across the province, Ontario Health, the Ministry of Health and relevant internal/externals stakeholders to advance the implementation of the OHT Strategic Plan and Annual Operating Plan.
  • Responsible for the development of a human resources plan, and on-going growth and development of CCHOHT staff members.
  • Support and enable the CCHOHT team and working groups, including diverse stakeholders to construct and carry out workplans with project objectives, scope, accountabilities and timelines.
  • Work with Patient Advisor leads to enable patients, caregivers, and families to actively participate in all of the CCHOHT's activities, while ensuring all aspects of the CCHOHT are guided by the Patient Declaration of Values for Ontario.
  • Coordinate with other Ontario Health Teams and regional health planning bodies to learn and apply emerging best practices for OHT's.
  • Accountable for all fiscal activities including, but not limited to budget development, expenditure planning for operational and project funding, reporting to the Collaborative Committee as well as partners such as Ontario Health, as required.
  • Facilitate the development of proposals for operational and capital funding from the Ministry of Health or other related opportunities that will support the work of the CCHOHT.
  • Develop and implement an annual communications strategy in support of the Strategic Plan and Annual Operating Plan that reflects the needs of the CCHOHT, and many diverse stakeholders, including patient and family caregiver partners, Ontario Health and the Ministry of Health.
  • Ensure the CCHOHT is well-represented on local, regional, and provincial initiatives related to its work.

Education and Expertise

  • Minimum 10 years of progressive leadership in health management experience, preferably in a range of care settings.
  • Post-Secondary Degree (Masters required) in a health-related field, business or equivalent.
  • Experience managing and providing oversight to clinical programs and/or frontline clinical experience preferred.
  • Experience in the continuum of care across large and small communities, with knowledge of considerations for these sectors in the Halton Region.
  • Experience working directly with health care providers, including physicians and other primary care providers.
  • Proven success in vertical integration across the healthcare system; highlighting innovation and the design of integrated models of care.
  • Demonstrated experience in partnership development, including engagement and partnership contracts, an asset.
  • Outstanding skills in building strategic and productive relationship and developing and managing partnerships across functional departments and between organizations.
  • Strong understanding and analysis of population health methodology, health care utilization data, integrated care, and the social determinants of health.
  • Demonstrated experience in leading engagement of large, cross-functional teams, across organization boundaries and asset.
  • Proven experience developing and implementing project plans, programs, support tools and processes as well as corporate level project and program reporting.
  • Demonstrated experience working with, and knowledge about, marginalized populations, especially (but not limited to) Indigenous, Francophone, or racialized groups.
  • In-depth knowledge of local health issues, planning frameworks, and the Halton Region context.
  • Significant knowledge and understanding of the relevant legislation pertaining to the organization and related stakeholders.
  • Excellent communication skills are required: public speaking, written reports, and verbal presentations will be frequent activities.

Salary Range: $150,507 - $188,135 annually
