As a Master Social Worker, you will play a crucial role in providing comprehensive social work services to patients and their families, ensuring they receive the support and resources necessary for optimal well-being. Responsibilities:

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental factors impacting their health.
• Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive care plans that address the psychosocial needs of patients.
• Provide counseling and support to individuals and families dealing with chronic illness, end-of-life issues, and other health-related challenges.
• Advocate for patients' rights and access to appropriate resources, ensuring they receive the highest quality of care.Collaborate with community agencies and organizations to connect patients with relevant social services, financial assistance, and other support systems.
• Participate in case conferences, team meetings, and other collaborative efforts to enhance patient care coordination.Document all social work interventions, assessments, and consultations accurately and in a timely manner.

Qualifications:

• Minimum of 5 years of experience as a Master Social Worker, with a strong background in healthcare settings.
• Knowledge of medical terminology, healthcare systems, and understanding of the impact of illness on individuals and families.
• Strong communication and interpersonal skills with the ability to work effectively within a multidisciplinary team.
• Demonstrated ability to manage complex cases and work with diverse populations.
• Excellent problem-solving skills and the ability to make sound judgments in a fast-paced environment.
• Proficient in using electronic medical record (EMR) systems.
