Seniors Community Connector: Smithers/Telkwa area
Houston Link to Learning is looking for a Seniors Community Connector for the Smithers/Telkwa area. The connector will work with older adults who are referred from health professionals and community services to connect them with community support and services through referrals, applications, advocacy and introductions.
Qualifications and Qualities required:
A diploma or degree in Human Services or a related field would be an asset, or a combination of experience and education.
Knowledge of senior's issues and challenges related to healthy aging would be an asset or a willingness to take relevant training.
Experience working with older adults would be an asset.
Experience working with diverse populations from different cultures and socio-economic backgrounds.
Strong collaborative skills and proven ability to establish and maintain effective working relationships with all internal and external contacts.
Knowledge of local community resources, programs and services.
Strong communication, public relations, and interpersonal skills.
Strong English skills.
Strong organization, coordination, research, and time management skills.
Ability to be flexible and adapt to different situations in order to meet client and program needs.
Able to work independently or as part of a team.
Good computer skills, including Microsoft office.
Valid Driver's license and access to their own transportation.
Position term:
Part time, permanent, subject to funding.
Starts: As soon as possible to Mar 31, 2026, subject to probation and funding.
Please note that this position can be combined with the SCC position covering Houston/Topley to create a full-time position for the right candidate.
Salary:
Commensurate with experience
Starting at $24.00 per hour

Seniors Community Connector: Smithers/Telkwa area
The connector will work with older adults who are referred from health professionals and community services to connect them with community support and services. Connections will be made through referrals, applications, advocacy and introductions.
The goal of the position is to assist older adults to age safely in the right place by providing them with the social connections they require and build and maintain partnerships and connections of community-based seniors services in our area.
Key duties and responsibilities :
1. Community Development
Build and maintain networks within community and strengthen relationships within the community-based senior serving sector.
Mobilize individuals and organizations to provide better support for older adults.
Engage and participate in learning opportunities such as communities of practice, planning tables or networking events/meetings.
Assessment and management of referrals.
Connect, liaise and establish partnerships with local health care professionals to create and maintain referral pathways.
Maintain an active caseload of older adults through referrals.
Prioritize referral to meet individual participant needs.
Follow safety protocols when visiting seniors in the community, particularly in their homes.
Complete the intake process with participants to assess strengths, needs, abilities and risks using motivational interviewing techniques including active listening, conflict resolution and observing behavior.
Maintain a high level of confidentiality at all times.
Wellness Plan Development
Supports seniors to access an appropriate range of activities and community resources by developing individual wellness plans.
Using a 'what matters to you' approach refers to services and observe and assess the participants' engagement. Modify where necessary to meet the changing needs.
Provide information on local resources to socially support the participant and their family.
Assists with connections to a primary care provider and local health services.
Engages in educational training about healthy aging.
General
Documents participants interactions, wellness plans, reports and other administrative duties as required.
Participates in evaluation of the program.
Applies reflective practice to work
Maintains all statistics including contacts and phone calls.
Maintains a daily descriptive journal of all activities.
Attend job related in-service workshops and meetings as required including staff meetings.
Vacation and other coverage where required.
Other tasks as required or as assigned.
