Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in-home and school-based services to children, youth, families within the community.

Lynwood Charlton Centre is the Lead Agency for the Hamilton service area for Moving on Mental Health. This is a Ministry initiated plan to transform the experiences of children, youth and families dealing with mental health issues, and strengthen the community-based system for delivering mental health services to them. Through our Lead Agency role, we are responsible for leading and facilitating system reform in Hamilton. To do this, we work collaboratively with other child and youth mental health service providers, other affiliated community partners including our child welfare, health, and school board partners, and parents and youth engaged in our services.

The Centre is funded and licensed by the Ministry of Health (MOH) and the Ministry of Children, Community and Social Services (MCCSS) and accredited by the Canadian Centre for Accreditation. We are currently looking for Permanent Full Time Resource Worker to join GPS Integrated Mental Health Treatment Program.

Role Overview

Working under the direction of the Manager - GPS Integrated Mental Health Treatment & Specialized Counselling/Therapy Programs, the Resource Worker is responsible for the provision of evidence informed treatment interventions and mental health services to children/ youth and families in their schools, homes, and community. The Resource Worker will assess and plan treatment as well as develop effective relationships with service providers in the community.

Main Duties and Responsibilities

Provide direct service/counselling to the child / youth and or family in their school, home and/or community.

Administer the ChYMH assessment and employ a systemic approach to develop a strengths based treatment plan which provides a wide range of evidence informed treatment interventions appropriate to the needs and strengths of the child/youth and family by:

o Utilizing relevant intake and service history

o accessing both formal and informal resources

o utilization of standardized assessment recommendations

o regular supervision

o access to consultation

Provision of information and education to children/youth, families, and school board collaterals to support the implementation of the treatment plan.

Maintain clinical records, assessments, and reports according to agency and program policy and procedures.

Contribute to the overall case planning as generated by the child/youth, family, school board collaterals and the interdisciplinary team.

Utilize referral information and explore the child/youth and family's previous involvement in community services to inform case planning

Support and provide advocacy for the child/youth and family in accessing and/or making referrals to community resources.

In collaboration with the Social Workers, liaise, facilitate and participate in meetings with community partners.

May be required to contribute to the field's development by supervising a student on placement.

Experience /Skills required

Diploma in CYC or Equivalent from recognized community college or a BA in a related field

Certification in CHYMHs (Child and Youth Mental Health) assessment or certification obtained within 3 months of hire

First Aid

Knowledge of child and youth mental health and relevant evidence informed treatment interventions

Several years of progressive experience working with families and children and/or youth in a treatment/counselling setting.

Experience working with and developing intervention plans aimed at enhancing client functioning.

Experience working with children/youth.

Experience collaborating with an interdisciplinary team to set and achieve common goals.

Experience with navigating and/or collaborating with other community systems

Other requirements

Broad Records Check done within the last four weeks.

Standard First Aid & CPR Level C/AED
