About this team:

Our specialized 16 bed inpatient reactivation program is located on the beautiful and quaint UHN-RCC campus. The Sinai Health RCC program is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. You will collaborate with a high performing multi-disciplinary team including Registered Nurses, Registered Practical Nurses, Nurse Practitioner, OT, PT, SW, and SLP to provide high quality and safe patient care.

In this role you will:

Possess the requisite knowledge, skills, judgment and attitude to deliver competent and evidence-based Occupational Therapy care
Complete assessment, planning and therapeutic intervention for an assigned patient caseload according to Occupational Therapy scope of practice
Documentation in electronic health record
Supervision of Occupational Therapy Assistants as per College Standards
Demonstrate accountability and responsibility for your decisions and actions and practices within your own level of competence
Participate in Interprofessional rounds and discussions
Continuously gather and analyze clinical and other relevant data to evaluate the effectiveness of the plan of care and adjusts the plan as needed
Work collaboratively with patients, families, caregivers and the interdisciplinary healthcare team
Provide health education using effective teaching strategies that takes into account the patient/family's learning needs, readiness, and motivation to learn
Contribute to discharge and transition planning and other planning activities to facilitate care continuity and coordination; advocate for care needs to be met in most appropriate setting
Utilize critical inquiry, critical thinking and research to develop evidence-informed practice and inform decision-making
Develop expertise in your area of practice through self-reflection, self-learning, specialty certification and implementation of best practice guidelines.
Be committed to patient safety and quality responsibilities in accordance with hospital vision, policies and procedures
Contribute to achieving Sinai Health's strategic and operational priorities through a commitment to continuous quality improvement, patient/staff safety, professional development and continuing education
Contribute to Sinai's work by participating actively in hospital committees and other groups to represent the OT perspective
Assist with other department/unit responsibilities or duties, as required
Mentor colleagues and new hires as appropriate
Participate in teaching of students from affiliated institutions.

Job Requirements

Job Requirements:

Mandatory

Currently registered with the College of Occupational Therapists of Ontario in good standing

Preferred

1 years of recent clinical experience
Experience using an electronic health record
Certification in Functional Independence Measure (FIM)
Certification in Montreal Cognitive Assessment (MoCA)
ADP Authorizer

Skills and Knowledge

Excellent health and clinical assessment skills
Patient handling, cognitive and functional assessment skills inclusive of objective assessments
Strong disposition planning skills and knowledge of community supports
Demonstrated ability in providing quality patient-focused care and addressing the fundamental care needs of patients and their families
Proven ability to make sound professional judgements and practice decisions and demonstrate effective time-management and organizational skills in a fast-paced work environment
Demonstrated leadership skills
Excellent problem solving and critical thinking skills
Excellent oral, written and verbal communication and interpersonal skills
Proficiency in English
Demonstrated ability to work effectively and collaboratively with others including patients and their families
Demonstrated satisfactory job performance and attendance history
Computer proficiency and/or Basic to Intermediate Microsoft Office skills (e.g. Word, Outlook)
