Position Description

POSITION SUMMARY
The Nurse Practitioner (NP) is a leadership position within Firestone Ambulatory Clinics. The NP will ensure timely, equitable access to a full range of high quality, integrated services, for individuals and families within Hamilton regions. The NP will act as the nurse navigator for the Respiratory Rapid Assessment Clinic (RRAC) and will provide support to patients and families receiving care and will facilitate access to services as needed. The NP will be responsible for providing expert consultation, facilitate access to care, services, and resources for the Department of Respirology and the Firestone Ambulatory Clinic patient population. The NP establishes and maintains linkages internally, and externally with community agencies and organizations, while fostering effective working relationship with external partners (such as the Six Nations Clinics, St. Joseph's Healthcare System, Brantford Community Health Center).
A strong communicator, the incumbent initiates and maintains communication with individuals and groups regarding key issues related to Department of Respirology RRAC within the Firestone Ambulatory Clinics. The NP will ensure successful navigation of patients from the point of initial or external referral, necessary diagnostic tests, development of care plan and discharge to subsequent care. Responsibilities will include triaging referrals with Physician support, coordination of diagnostic testing/investigations, attend consultation appointments and track results while diagnosis and plan of care are developed and implemented. Thorough documentation is required for all encounters with the patient. The NP will link individuals and families with practitioner and support services and will be liaison between the patient, family; clerical support and clinicians to ensure timely and seamless care. The NP will be directly working with Respirologist in this role. The NP will oversee and liaise with clerical staff to ensure seamless patient flow and the completion of appropriate testing and patient booking. Leading weekly clinics including remote care is also expected.

QUALIFICATIONS
• Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (RN-EC) with specialty certificate NP-Adult required
• Master's degree in nursing required
• Member RNAO; NPAO membership preferred
• Minimum three years' experience as an Acute Care or Primary Care Nurse Practitioner required
• Recent, within 2 years, knowledge of Thoracic, Respirology (medicine, surgical) nursing experience required
• Demonstrated clinical leadership experience preferred
• Current, expanded knowledge regarding respiratory disease care paths required and the related knowledge of the organizations, systems and services preferred
• Strong clinical skill/knowledge to include, but not limited to: Alaris infusion pumps, venipuncture, IV therapy and phlebotomy, thoracentesis procedure support, sterile technique
• Strong interpersonal and facilitation skills in internal and external environments
• Ability to establish credibility with, and gain consensus among individuals and groups with varying interest
• A collaborative approach and the ability to function effectively as a part of a team
• An enthusiasm for quality improvement in healthcare
• Demonstrated ability to develop, implement, and sustain community partnerships required
• Demonstrated ability to manage programs and projects required
• Excellent organizational skills with the ability to meet multiple deadlines
• Effective communication skills, both verbal and written and interpersonal skills
• Patient education skills with family and patients
• Excellent presentation, public speaking, and facilitation skills
• Strong working knowledge of EPIC (Dovetale), Word, Excel, Power Point, and Outlook
• Demonstrated strong organizational, critical thinking and problem-solving skills
• Ability to function as an effective member of a collaborative interdisciplinary team
• Smoking Cessation Counselling training preferred
• Must have a good attendance and work record

RESPONSIBILITIES
• Provides advanced evidence-informed clinical care to patients of Firestone Ambulatory Clinics (Respirology and Ambulatory Clinic based patient population)
• Develops/revises evidence-informed policies and procedures in collaboration with nursing and interprofessional team members
• Participates in planning, implementing, maintaining, monitoring, and developing reports for appropriate database(s) for measurement and support the evaluation process quarterly
• Identifies areas of improvement within Respirology and Ambulatory Clinic practice to promote ongoing quality assurance, and plan, implement, evaluate and report on outcomes of improvement initiatives
• Develops, implements, and evaluates educational initiatives to support Respirology and Ambulatory Clinic nursing practices.
• Leads in the implementation and sustainability of best practices within Respirology and Ambulatory Clinic based patient population.
• Initiates and actively participates in strategic planning and program development and evaluation
