Position Summary

Reporting to the Clinical Manager of the Centralized Resources Team (Post-Acute Float Pool), the successful applicant will be a part of the Specialized Pod supporting the Post-Acute and Integrated Stroke Units of the BCHS. The Post-Acute Pod includes the Rehabilitation, ALC, Medically Complex/Palliative and Transitional Units in Brantford (Brantford General Hospital) and Paris (The Willett site). Our Post-Acute units provide compassionate specialized care and inpatient services for patients and families who have completed their acute healthcare experience. Patients will be supported in their needs in anticipation of a return to a community setting (home or other residential living), or to receive palliative and end of life care.

The Registered Practical Nurse, under the Standards of Practice of the College of Nurses of Ontario functions as a member of the nursing team to provide preventative, restorative, educational and/or supportive care. Excellent knowledge, critical thinking, decision making and communication techniques are employed in assessing, planning and implementing nursing care

Mandatory Qualifications

Graduate of an accredited Practical Nursing program
Current registration in good standing with the College of Nurses of Ontario
Current Basic Cardiac Life Support (BCLS) certification
Gentle Persuasive Approaches (GPA) training an asset
Effective September 7th, 2021 all applicants must provide proof of two doses for full vaccination against Covid-19 (in the form of watermark receipts from the provincial portal) or provide written proof of a medical or religious reason for not being fully vaccinated as per Organization Policy, Covid-19 Immunization and Management.

Mandatory Competencies

Experience with caring for older adults and patients with medical conditions, cognitive impairment and/or responsive behaviors in hospital, community or other congregate care settings is an asset.

Must be able to function within the full scope of practice as a RPN

Preferred Competencies

Demonstrated competence in patient assessment and care planning
Member of RPNAO or other professional association is an asset
Reliable individual with excellent attendance record and able to work all shifts
