Overview CarePartners is embarking on a new program in partnership with the Home and Community Support Services (HCCSS) Central East (Haliburton Region) and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).What Makes this Position so Exciting?In this exciting staffing model, HISH CSWs will have consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends:Two-week RotationMon/Tues/Sat/Sun0800 - 2000 hrsWed/Thurs/Fri0800 - 2000 hrsThis is an equivalent of seventy-seven (77) hours per bi-weekly pay period!
