Overview CarePartners is embarking on a new program in partnership with the Home and Community Support Services (HCCSS) Central East (Haliburton Region) and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).What Makes this Position so Exciting?In this exciting staffing model, HISH CSWs will have consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends:Two-week RotationMon/Tues/Sat/Sun0800 - 2000 hrsWed/Thurs/Fri0800 - 2000 hrsThis is an equivalent of seventy-seven (77) hours per bi-weekly pay period! What We Offer 77 hours bi-weekly pay A special $500 to $750 bonus for signing with CarePartnersPAID MILEAGE at 0.45 per/kmPaid orientation and hands-on mentoring from experienced, caring staffAccess to group discounts, including exclusive discounts from various retailers Total rewards program which includes health & dental benefitsA robust Employee Assistance Program (EAP) for you and your family membersOpportunities for continuing education and trainingHands on mentoring and leadership supportOpportunities to volunteer in countries with limited access to healthcare services What The Role Involves Contributing as a valued member of the interdisciplinary care teamAssisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functionsPromoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safetyProviding cognitive, social and/or emotional support to the patientPreparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practicesProviding patient status reports and participate in patient/family care planning meetings What You Bring Community Support Worker (e.g.) Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program) education, with preference given to those with a PSW certificate and/or equivalencyA valid driver's license and vehicle for which you are insuredA strong desire and commitment to making a difference in the lives of their patientsExceptional interpersonal skillsAbility to work both independently and as part of a teamExperience working with patients with significant and complex care needs such as dementia and mental healthTo protect our patients, we require current vulnerable sector check screening, a clear background check and proof of COVID 19 vaccination* CarePartners In Your Community In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
