Hiring Manager: Manager, Youth Wellness Hub

Primary Location: Oshawa South, Ajax Youth Wellness Hub and across various agency locations within Durham Region.

Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive not only sick care, but preventive care as well.

At DCHC, we are a team-based interprofessional group of staff including physicians, nurse practitioners, nurses, counselors, dietitians, outreach workers, medical secretaries, and other administrative staff. Position Overview


The Intake Coordinator/Care Navigator plays a pivotal role in ensuring that youth accessing the Hub experience a seamless, welcoming, and youth-centered entry into services. Using the YWHO intake processes and standardized assessment tools, this role gathers relevant information to determine service needs, supports measurement-based care, and facilitates warm transfers to Hub programs, partner agencies, and external service providers.

The Intake Coordinator/Care Navigator works closely with youth to support engagement and continuity of care, liaises between Hub staff and community partners, and contributes to trauma-responsive, culturally safer, and youth-centered service delivery.

Key Responsibilities

Intake & Assessment

  • Conduct intake interviews with youth using YWHO standardized forms and procedures.
  • Collect information to determine appropriate service pathways.
  • Explain measurement-based care to youth and support them in completing assessments.
  • Ensure intake processes are trauma-responsive, culturally safer, and youth-centered.

Care Navigation & Follow-up

  • Facilitate warm transfers to Hub programs, partner agencies, or external service providers.
  • Support youth in maintaining engagement and continuity of care.
  • Track follow-ups and outcomes, advocating for youth as needed.
  • Liaise between youth, Hub staff, and external providers to ensure coordinated care.

Partnerships & Network Engagement

  • Build and maintain strong relationships with schools, community agencies, and youth-serving networks.
  • Participate in collaborative initiatives and community tables to strengthen the Hub's referral and support network.
  • Support identification of service gaps and opportunities for improved youth access.

Documentation, Data & Quality Improvement

  • Maintain accurate, confidential records of intake, assessments, referrals, and follow-ups.
  • Participate in data collection, reporting, and continuous quality improvement activities.
  • Monitor key performance indicators (KPIs) aligned with Hub, funder, and YWHO expectations.

Collaboration & Safety

  • Work collaboratively with Hub staff, including CYW/Facilitator, Youth Outreach Worker, Peer Support Worker, to ensure integrated, youth-centered care.
  • Adhere to health, safety, and crisis response protocols.

Qualifications

  • Diploma or degree in Social Work, Social Service Work, Child and Youth Care, or related field.
  • Knowledge of youth mental health, substance use, wellness services, and community resources.
  • Minimum 2 years experience with care navigation, intake, or assessment processes, preferably in a youth-serving context.
  • Understanding of trauma-responsive, culturally safer, and anti-oppressive approaches.
  • Excellent communication, relationship-building, and advocacy skills, with ability to engage youth and families.
  • Ability to build and maintain collaborative relationships with external partners and service providers.
  • Familiarity with measurement-based care, YWHO intake tools, and standardized assessment forms is an asset.
  • Proficiency with data entry, reporting, and quality improvement tools.
  • Valid Ontario driver's license and unlimited use of a vehicle is a requirement
  • Flexibility to work some evenings and weekends to support youth access.
  • Satisfactory criminal reference and vulnerable persons check
