Hours of Work: 8:00-4:00 Monday-Friday

Reports to: Dr Matthew Piamonte, Physician Lead

Job Summary

The Family Practice Nurse Practitioner will work in close collaboration with physicians and other interdisciplinary health professionals in a community-based collaborative care clinic. As a vital member of the team, the Nurse Practitioner is responsible for delivering high quality comprehensive primary care to patients with a focus on the assessment and management across the full spectrum of primary care presentations in all populations, as well as health education, disease prevention, and chronic disease management.

Key Responsibilities

• Manage a family practice roster of 500 patients independently in year 1
  • Expectation to increase roster to 800 patients total during year 2
• Cross-coverage of parallel practices while other providers are absent.
• Focused engagement in quality improvement initiatives and performance tracking that aim to improve delivery of care.
• In collaboration with other providers:
  • Create and maintain up-to-date patient charts.
  • Review incoming investigations.
  • Primary care acute/urgent issues assessment and management.
  • Chronic disease management and surveillance.
• Opportunities for engagement on particular patient panels of interest, including (but not limited to) acute care, sexual health, mental health, perinatal health, pediatric medicine, etc.

Working Conditions

The NP position requires availability to work in the clinic during regular working hours from Monday to Friday. Cross-coverage expectations, after-hours clinics, virtual care/work from home clinics, and/or occasional meeting attendance outside of office hours are negotiable.

Qualifications

• Bachelor's Degree in Nursing and successful completion of a postgraduate Nurse Practitioner program in Primary Health Care.
• Registered as a Nurse Practitioner in good standing with the College of Nursing of New Brunswick.
• Current CPR or Basic Cardiac Life Support (BCLS) Registration.
• Minimum of 3-5 years' experience providing patient care to patients of all ages in a comprehensive Family Practice or relevant Ambulatory care setting.
• Current formalized physician consultant role (note: employer can provide same).

• Previous experience with an electronic medical record (EMR) program is an asset.
• Knowledge of the NB primary health care system is an asset.
• Bilingualism is an asset.
