Definition of Work:
Reporting to the Director of Patient Flow and Site Operations. The Acute Care Nurse Practitioner (NP) works collaboratively with the hospitalist service, site lead physicians and interdisciplinary team members. The NP participates in interdisciplinary patient care planning, handover discussions, collaborative inpatient management, patient assignment and workload coordination, and discharge planning activities in partnership with the hospitalist service and healthcare team. The NP’s primary role provides clinical care to inpatients at James Paton Memorial Regional Health Centre, with presentations that include various acute and chronic health conditions. Operational requirements may necessitate support in other clinical areas (including but not limited to personal care homes, long-term care and community settings) consistent with the NP’s competencies and organizational need. The successful candidate will work collaboratively with unit-based nursing staff, leaders, allied health team members, subspecialists, the Hospitalist Team and administration to provide comprehensive, holistic, patient-family centered care. The NP will participate in the regular morning huddle with the site-operation team and work with the multidisciplinary team to facilitate patient flow and discharges.
The Nurse Practitioner is expected to perform advanced comprehensive health assessments; order, interpret and perform diagnostic tests; provide input related to primarily ALC and medically discharged patients; however, may need to manage active inpatients within their scope of practice in acute care settings in collaboration with both hospitalist and other providers. The successful candidate will communicate assessment findings, diagnoses, including outcomes and prognoses; prescribe appropriate pharmacological, medical, and other therapies; utilize complex reasoning to safely coordinate and manage patient care while promoting continuity across the continuum of care.
The position will contribute to collaborative inter-professional patient care through advanced clinical practice, consultations, referrals, leadership, education, research, and other related duties as required. This position will require strong
interpersonal skills, the ability to build collaborative, trusting relationships and will demonstrate skills in the organization, conflict resolution, advanced problem solving, critical thinking, change management and interdisciplinary leadership. A passion for caring and advocating for patients with advanced medical complexity, and their families, is essential.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
