DEFINITION OF WORK: The Licensed Practical Nurse I is an integral member of the FACT Team that provides intensive mobile services to individuals with serious mental illness and concurrent related complexities of mental health issues/disorders. With team members and the client, this position is responsible for providing comprehensive client assessments and support to individuals in establishing personal recovery plans, as well as short and long term goals. The LPNI will provide practical mental health nursing and therapeutic care in the assessment process, assists with implementation of care plans, collaborate with members of the multi-disciplinary healthcare team, and contribute to the treatment of clients. The successful applicant will consult and work in partnership with local partners, service providers, and community agencies in the provision of service to clients of the FACT team. This work is performed independently with strong links to community stakeholders and other staff within the FACT team.
REQUIRED QUALIFICATIONS: Competition of high school or equivalent, as well as graduation from an approved practical nursing program and current registration with the College of Licensed Practical Nurses of Newfoundland and Labrador are required. The ability to develop helpful relationships and communicate clearly and effectively with others is required. Experience in the area of Mental Health and Addictions including supportive counseling, assessment and crisis intervention is an asset. Please Note: The use of a private vehicle and a valid driver?s license is required for this position.
While primary responsibility is with the Western Zone FACT teams, incumbent may also be required to support individuals from other areas of Mental Health and Addictions Program.
