Job Description
The Community Health Nurse II (CHNII) position within the Health Promotion Non-Communicable Disease & Injury Prevention core program, provides advanced knowledge, leadership and coordination for Health Promotion within the Provincial Public Health program which will focus on health equity, as a priority for communities and groups that are experiencing disproportionate negative effects related to social determinants of health and are at risk of poor health outcomes. This advanced nursing role focuses on applying evidence-informed practices and the Provincial Public Health Framework to promote health equity within the Provincial Public Health Program. The CHNII will support and consult with the public health team provincially, across all zones of NLHS.
Key responsibilities include (but are not limited to):
- Leading a comprehensive population health approach in the assessment, planning, development, coordination, implementation, and evaluation of health promotion/ public health policies, programs, initiatives, services, and resources.
- Accessing and critically appraising data and evidence (i.e. published and grey literature, systematic reviews, and promising practice).
- Using accepted models of practice and evidenced informed research to guide the development of health promotion strategies, programs and initiatives
- Collaborating with NLHS staff, community partners, and people with lived experience to build strong relationships to support the work to advance priorities
- Identify and develop resources and program materials related to public health priorities.
- Participating in various local, zonal, provincial and national committees and coalitions to foster knowledge translation of evidenced informed research and support healthy public policy development.
- Assessing, designing and implementing education and training programs to address public health priorities for staff and community partners based on identified needs.

-Occasionally lifting and moving resource material such as audiovisual equipment and workshop materials between 10 - 25 lbs.
-Regularly sitting to a computer, standing/walking or driving to and from sites to participate in workshops and training sessions in the zone where there is repetitive alertness required.
-Regularly operating a computer, performing fine finger or precision work where visual concentration is required for reading text, performing data entry and compiling statistical reports.
-Regular auditory concentration is required for routine verbal communication.
-Time pressures and deadlines to fulfill work activities, complete reports and to respond to funding requests.
-An occasional lack of control over the pace of work due to the requirement to participate in community meetings and committee work.
-Occasionally work results in fatigue, requiring periods of rest.
Qualifications:
- Bachelor's degree in nursing and current registration; a master's degree in public health or Health Promotion is an asset.
- Minimum of two years' experience in community-based health promotion including the application of population health and health promotion theory and strategies; retrieval and analysis of health status information; and identifying appropriate health promotion actions, is required.
- Strong leadership, communication, collaboration, facilitation, and strategic planning skills.
- Knowledge of health equity, adult learning principles, and public health competencies.
- Experience in community mobilization and collaboration/partnership development.
- Valid driver?s license and access to a vehicle; travel required.
- Criminal record check required.
