We are searching for a Full-time Manager of Transitional Services to join our West BedfordTransitional Health team based in West Bedford, Nova Scotia.
Through an innovative partnership with Nova Scotia Health (NSH), Shannex proudly operates the province's first transitional health facility in West Bedford. This facility provides specialized care for NSH patients in the central zone who no longer require acute hospital care but need additional care to safely return home or transition back into their community. Our team is committed to dignity, safety, and clinical excellence in every interaction.
The Manager of Transitional Services will lead the operations of assigned inpatient units, ensuring high-quality, patient-centered care that promotes independence, functional recovery, and timely discharge. This position plays a key role in improving patient flow, reducing length of stay, and enhancing overall patient experience.

About the Opportunity
- Leads and supports a respectful, inclusive, and accountable team culture
- Oversees recruitment, onboarding, coaching, and performance management
- Provides clinical leadership to nursing staff and interdisciplinary teams
- Advances the transitional health model through innovation and continuous improvement
- Leads quality improvement, risk management, and infection control initiatives
- Manages policies, procedures, and the operating budget to ensure safe and efficient care
- Collaborates with acute care partners to support timely admissions and safe discharges
- Monitors EDDs, length of stay, readmissions, and discharge to home outcomes
- Champions a Home First philosophy
About You
You are a collaborative healthcare leader with a passion for quality care and system improvement, and you bring:
- Bachelor of Nursing or Bachelor of Science in Nursing
- Current registration with the College of Registered Nurses of Nova Scotia
- Minimum five (5) years of nursing experience, including leadership responsibilities
- Valid CPR and First Aid certification
- Demonstrated ability to lead teams and manage performance
- Understanding of budgeting and basic financial principles
- Ability to complete a Criminal Record and Vulnerable Sector Check
Assets:
- Master of Nursing
- French/English bilingualism
- Experience leading in an inpatient or unionized healthcare environment, with a focus on care transitions and Home First principles
