Position Scope:

Under the direction of the Assistant Director of Resident Care/Director of Resident Care and the President/CEO, the Coordinator of Care is responsible for managing the nursing program within Mountain Lea Lodge (MLL), in accordance with the vision, mission and values of the Mountains & Meadows Care Group. The Coordinator of Care is accountable for overseeing that program directions are carried out. This will include direct supervision and management of the Long Term Care Assistants (LTCA), Continuing Care Assistant (CCA), Licensed Practical Nurse (LPN), and Registered Nurse (RN)/ supervisors within the facility. The Coordinator of Care will assist the Assistant Director of Resident Care to provide educational opportunities for all staff. The Coordinator of Care works closely with the Assistant Director of Resident Care to promote consistent, quality support and care to the residents who live within Mountain Lea Lodge.

Organizational Relationships:

· Effectively manages, supervises, and supports staff within his/her portfolio.

· Maintains a supportive, caring, professional relationship with residents and their families (or substitute decision maker)

· Maintains a positive working relationship within the Management Team of MLL.

· Develops and maintains positive working relationships with the Assistant Director of Care and any other agencies involved in supporting residents.

· Establishes and maintains a positive working relationship with the Recreation Manager for MMCG

· Communicates regularly with the Assistant Director of Resident Care, Registered and Licensed Practical Nurses, LTCA, CCAs to deliver consistent, professional, quality support to residents based on evidence and best practice.

· Accountable for overseeing resident movement (admissions, discharges, and transfers as well as bed reallocation/room transfers)

· Accountable for investigation and follow-up of all nursing program incidents.

· Remain fully conversant with the current Collective Agreements along with the organization's policies

· Participates in meetings (especially, Pharmacy and Therapeutics) and educational opportunities.

· Helps in policy development as requested

Key Activities:

· Establish and maintain effective systems to meet resident needs for exceptional quality programs.

· Has input in the development of programs that meet the social, recreational, spiritual and personal needs of the residents

· In collaboration with the Registered nurse, ensure resident care plan and review meetings occur in a timely manner.

· Weekly phlebotomy

· Accompanies doctor during weekly rounds

· Acts as liaison with Pharmacy, etc.

· Organizes and assists with wound care and monthly/PRN wound care rounds

· Willing to perform other duties as assigned by the Assistant Director of Resident Care.

· Assist, support, and guide residents in all activities of daily living including, but not limited to: recreational and leisure activities; provide meal and snacks as required; and maintaining the general upkeep of the home.

· Maintain a safe and supportive environment for all.

· Document and/or communicate changes in resident mood, behaviour, or condition to all appropriate staff.

Qualifications:

· Currently registered as an LPN with the College of Nurses of Nova Scotia

· Minimum of three years of progressive nursing experience and education in which leadership, administrative and interpersonal skills have been demonstrated.

· Current phlebotomy course.

· Must have excellent communication skills both verbal and written.

· Must have excellent conflict management skills.

· Able to work with different collective agreements and in a unionized environment

· May require flexibility with work hours at times

· Current certification in CPR and Standard First Aid

· Training GPA

· Teaching experience an asset

Ability to work independently

Excellent communication (written and verbal) and conflict management skills

Ability to develop, implement and evaluate resident-centred programs

Proven organizational and time management skills

Ability to multi-task and work in a fast-paced environment

Demonstrated computer skills, preferably in MS Office Suite

Work Environment:

Usual work environment is indoors.

Pets and houseplants may be part of the work environment

Additional:

Must be able to maintain regular attendance

This is a union position and salary is per current NSNU Collective Agreement
