We are searching for a Client Medication Assistant to join our PARKLAND AT THE COMMON team based in HALIFAX, NOVA SCOTIA.

About the Opportunity
- Assisting with planning and implementation of client recreation and leisure activities;
- Coordinates with CSC for prescription changes and refills with the client's pharmacy;
- Participates in and receives report daily. Plans and organizes daily work assignments, and prioritizes the specific needs of the clients;
- Communicates with the CSC if there is a noted change in the health status of the client e.g. decreases in appetite, increase in confusion, physical weakness, etc.;
- Develops an awareness of the specific needs of each client e.g. diet, allergies and recreation;
- Monitors clients assessed for at risk behaviors (exploring, falls, choking, etc.) and intervenes appropriately when necessary;
- Escorts clients to dining and activities as required;
- Completes domestic functions for clients according to the service plan including light housekeeping, dining assistance, and laundry;
- Demonstrates an interest in personal development. Participates on/in: department meetings, committees, internal and external education programs, other programs as required;
- Completes required documentation of work e.g. Daily Care Flow Sheet, Medication Administration Record (MAR);
- Communicates with health services staff throughout the shift to ensure continuity of care for clients;
- Ensures appropriate records are completed and maintained
- Performs all other duties as assigned.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- High School diploma or equivalent education completion;
- Continuing Care Assistant, Home/Personal Support Worker Program, Special Care Home Worker Program, Health Care Aid or Human Services Program, or equivalent;
- Parkland Medication Administration Certificate Program or equivalent
- Current CPR & Emergency First Aid Certification considered an asset.
- Basic Food Safety /Food Handlers Course considered an asset
- Alzheimer's Disease and other Dementia Care Course considered an asset.
- Completion of Back in Form Lift and Transfer Program considered an asset
- Ability to assess and prioritize multiple work assignments while displaying a high level of professionalism and commitment to quality service
- Adheres to the Standards of Nursing Practice, Code of Ethics, Registered Nurses Act and Regulations as defined by provincial legislation
- Maintain current knowledge of organizational policies and procedures including those that govern resident care
- Current knowledge of literature and applied research for the delivery of health care
- Current knowledge of community resources (e.g., services and programs), and roles of health care professionals
- Current knowledge of issues and priorities within health care with a focus on continuing care
