JOB PURPOSE
The Discharge Planning Coordinator works in collaboration with the patient and their family and/or support network, the nursing units and the interdisciplinary team to coordinate a safe, appropriate and timely discharge of the patient.
QUALIFICATIONS
- Baccalaureate Degree in Nursing
- Active Registration with the Nurse Association of New Brunswick (NANB)
- A minimum of 5 years clinical experience and has worked with interdisciplinary teams
- A strong knowledge base of community resources
- High regard for confidentiality, ethical comportment, and the ability to effectively deal with conflict
- Proven computer technology skills: Microsoft applications: Office, Word, Excel, PowerPoint; Allscripts and ability to use virtual meeting forums such as SKYPE®
- Must have the ability to work flexible hours
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record.
KEY RESPONSIBILITIES
Direct Patient Care
- Assessment of the patient's status and discharge needs
- Responsible coordinating tasks for patients requiring discharge planning services
- Works in collaboration with the patient and/or family and interdisciplinary team to ensure that the discharge plan is coordinated, comprehensive and updated as required through regular re-evaluation of the patient.
- Participation in relevant patient related activities, including but not limited to family meetings, interdisciplinary rounds and/or huddles.
- Provides patient teaching and support regarding the discharge process, referral agencies and community services.
- Identifies, documents, and communicates gaps in resources, potential delays and/or barriers to discharge. Establishes the intended outcomes for the discharge plan.
- Initiates and/or collaborates on any formal referrals that are required
- Prepares patient cases for submission to relevant review committees, such as Discharge Rounds and Integrated ALC
- Follows-up with the patient post-discharge to assess the effectiveness of the discharge plan and troubleshoot problem areas accordingly.
- Uses a variety of therapeutic communication techniques to interact and liaise with patients and families Collaborative Practice
- Is an active participant in Patient Flow by liaising with the Bed Access Coordinator and understands bed utilization trends and issues.
- Acts as a professional role model and/or mentor in Discharge Planning
- Is a resource for clinical teams on Discharge Planning and provides information on agencies and services
- Makes sound decisions and behaves in ways that reflect the professional values and services of the Discharge Planning Dept. when liaising with internal and external care partners Knowledge Translation
- Maintains an awareness of the current trends and issues affecting healthcare
- Strengthens professional self-development through continuing education.
- Stays up-to-date with knowledge of what is available and accessible in the community for resources and services.
- Utilizes best practices, research, standards, policies and procedures to inform Discharge Planning practices
- Collates, analyzes and interprets data collected and identifies potential gaps
- Prepares written reports and/or data submissions as requested
- Actively participates in the development of materials that support the Discharge Planning Dept. such as, patient information and dept. resource materials
- Develops and presents presentations on behalf of the Discharge Planning Dept. as required
- Any duties as assigned at the discretion of the Administrative Director
