Position: Continuing Care Assistant / Personal Care Worker
Department: Nursing Department
Responsible To: Director Resident Care

Position Summary: Is responsible to provide individualized care to each resident ensuring physical, social and emotional needs are met. Nursing services are provided in keeping with the mission, vision and values of the Home within an environment of safe practice, respect for dignity and resident focused care.

Responsibilities:

  1. Promote and maintain the safety of residents, staff and visitors as a as a strategic priority for the Home at all times
  2. Actively participated in a multidisciplinary approach to resident focused care
  3. Ensure the resident's care needs are met. This includes but is not limited to personal care, nourishment, toileting, mobility, safety and socialization
  4. Ensure the policies of the home are followed in the delivery of care.
  5. Ensure a clean, comfortable, safe and secure environment for residents and co-workers
  6. Assist the RN/LPN with the admission, transfer and discharge process
  7. Report all changes in resident health status to RN/LPN and directs all inquiries regarding care to RN/LPN
  8. Complete documentation in accordance with the Home's policies
  9. Adherence to food safety standards in the preparation of snacks and drinks
  10. Practice standard precautions and adheres to infection control Procedures
  11. Follows Infection Control procedures regarding the proper cleaning of equipment between resident use.
  12. Knowledgeable on WHMIS, Fire/Emergency Plan and use of fire equipment
  13. Reports and removes from service any unsafe /faulty equipment
  14. Ensure that the call buzzers function and respond to any calls during the shift.
  15. Maintain confidentiality and privacy of residents according to policy.
  16. Utilize good body mechanics when providing care
  17. Seeks/participates in ongoing educational opportunities

Qualifications:

  1. Grade 12 or equivalent
  2. Continuing Care Assistant certification

Other Requirements:

  1. Good general health which allows for the ability to attend work on a regular basis and meet the physical demands of the position
  2. Good personal hygiene
  3. Personal police record check clearance and negative Vulnerable Sector Check

Knowledge and Skills:

  • Ability to work independently and as a team member
  • Ability to cope with change and work demands
  • Demonstrated ability to communicate effectively with residents, family, staff and other health professionals
  • Organized and able to set priorities in a constantly changing work environment

Accountability:

  • Knowledgeable of and implements the values and mission of the home in the delivery of care
  • Knowledgeable of departmental policies and procedures
  • Practices and promotes occupational health and safety
  • Knowledgeable of Protection of Persons in Care legislation

Continuing Education Responsibilities:

  • Attends in-services, educational and training sessions for self development
  • Attends compulsory CPR, fire safety and WHMIS training as required
  • Attends and participates in departmental staff meetings

Hours of Work:

  • Shift rotation - required to work weekends, holidays and all three shifts
