The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client?s need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT?s will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers. The FCT for the Gander/ Gander Bay will include the geographical areas of Gander and Gander Bay.

Definition of work: As part of the collaborative team, the successful candidate will be expected to practice to their full scope of practice to meet client needs, participate in direct care of clients and provide clinic leadership. The FCT aims to provide continuity of care to improve health outcomes and quality of life through client attachment and client paneling to clinic providers, with the support and wrap around services of the full team. This position is a leadership role responsible for coordination of clinical client care as well as assisting clients and their families to navigate collaborative teams and community services. The Registered Nurse IIA will report to the Regional Manager for Family Care Team of the designated area.

The Registered Nurse IIA will conduct advanced client assessment using Electronic Medical Record (EMR) documentation and will lead others in the BETTER program approach and remote patient monitoring for chronic disease screening, management and prevention approaches including education, consultation, motivational interviewing, detailed assessment, investigation, follow-up, and evaluation within their individual and clinic case load. This position also involves the ongoing development, implementation and evaluation of comprehensive education programs for individuals with chronic diseases as well as their support systems. The nurse will work closely with the manager and other team members in program development, and client and clinician engagement. The role will encompass participating in clinic evaluations and continuous quality improvement processes based upon current best practice guidelines. Flexible hours are required (combination of days, evenings, and weekends required). Travel throughout the region may be required.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
? Undergraduate Degree in Nursing.
? Current registration with the College of Registered Nurses of Newfoundland and Labrador.
? A minimum of 3 years of experience within the last 5 years working in an adult acute care medicine/surgical setting, primary health care setting, or community nursing program
? Current BLS certification for health care providers from the Heart and Stroke Foundation.
? Current Health Assessment certification or willingness to complete the next available course.
? Demonstrated exceptional leadership qualities and problem-solving skills, as well as, oral and written communication skills.
? Demonstrated ability to work well independently and collaboratively with others in an interdisciplinary team environment and community.
? Demonstrated ability to be self-directed and exercise independent nursing judgment in a community-based environment with limited supervision.
? Demonstrated initiative.
? Demonstrated ability to develop, implement, and evaluate community health nursing programs and healthy public policy in a population health framework.
? Demonstrated ability to apply basic computer skills.
? Willing to work a flexible work schedule.
? A valid Newfoundland and Labrador Class V Driver's License.
? Consistent demonstration of NLHS corporate values; Innovation, Compassion, Accountability, Respect, excellence.
? A satisfactory record of work performance and attendance is required for this position.
? A satisfactory certificate of conduct from the RCMP or local police authority.

Preferred qualifications:
? Demonstrated skill in using health care-based computer systems (CRMS, Meditech, EMR).
? CNA certification in Community Health Nursing.
? Experience in the care and management of patients with diabetes, COPD, heart failure, and/or mental health disorders.
? Successful completion of an approved chronic disease management course.
? Successful completion of the Canadian Diabetes Educator (CDE) certification.
? Training as a BETTER practitioner.
? Previous experience in adult education.
