We are searching for a Continuing Care Assistant to join our Parkland at Home team based in Halifax, Nova Scotia.

About the Opportunity
- Assists clients with hygiene needs and daily living activities including bathing, dressing, oral care, skin care, positioning in bed, and transferring to and from bed to chair
- Assists with client's special needs, e.g. eye care, colostomy, and catheter
- Recognizes and reports a situation or change in resident circumstances, conditions or behavior which poses a threat to the client's health, safety, security and well-being
- Assists in restorative care e.g. walking, range of motion
- Provides mealtime assistance when required
- Provides snacks, morning and afternoon fluid passes and night lunches, while monitoring intake
- Collaborate with the client, client family and other members of the interdisciplinary team in the development of an individualized resident Care Plan
- Fills and replenishes supplies in client rooms, households, and washrooms
- Provides basic laundry services and seamstress support (e.g. sewing on buttons, applying clothing labels, etc.).
- Cleans rooms in client(s) private home after areas utilized for personal care, meal preparation and serving, etc.
- Utilizes supplies and equipment in a responsible way.
- Maintains privacy and confidentiality in relation to clients, client families, employees and visitors at all times.
- Documents according to facility policy e.g. Daily Care Record, progress notes, initiating medication incident reports, documentation and audits, incident reports etc.
- Communicates and reports pertinent and relevant information and/or observations to the Home and Community Support Manager(s).
- Reports all concerns to RN/LPN on site if in Retirement Living or Enhanced Care facilities.
- Follows safety, environmental and infection control practices as per Shannex policies.
- Completes rounds and corresponding documentation as per facility policy when in Retirement Living and Enhanced Care facilities.
- Additional duties as assigned/required
About You
- In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- Completion of Continuing Care Assistant program or equivalency according to the CCA Entry to Practice Policy
- Minimum 2 years experience in healthcare or home care setting.
- Proof of valid of the Food Handlers Certification (where full scope model applies)
- Proof of valid CPR/First Aid certification level C is required.
- Having the knowledge and ability to use computers and information technology to improve efficiency.
- Utilizing various modes of technology in a safe and ethical way. Includes the ability to learn and engage with emerging technologies and adapting quickly to new systems and software.
- Ability to manage and grow self through self-reflection and the development of exceptional interpersonal skills.
- Completion of Dementia: Understanding the Journey is considered an asset.
- Completion of Back in Form Lift & Transfer Program is considered an asset.
- Bilingualism in French/English is considered an asset.
- Class 5 driver's license and appropriate valid automobile insurance considered an asset.
