Job Summary
This is a professional Social Work position with primary responsibility for facilitating equitable and appropriate access to a broad range of Long-Term Care services for NL Health Services. Services include access to assessment and placement in nursing home, personal care home, and respite. These services are provided through assessing individuals, family/community needs and support systems; providing support, resource or adjustment counselling; consulting as a member of an interdisciplinary team on the development of care plans and evaluation of services.
The Social Worker liaises and consults with clients, families, members of the interdisciplinary team and other professionals in acute and long-term care, and other community settings. The position requires a high degree of responsibility, autonomy and accountability in assessing risk, responding to crisis situations and under time constraints, prioritizing work, making critical decisions in the context of limited resources and providing input to continuous quality improvement initiatives within the program. This work requires considerable professional judgment and is performed with a high degree of independence. The Social Worker is expected to perform in accordance with established policies and procedures of the organization. The Social Worker I will be responsible to provide Field Instruction to Social Work students and offer other clinical education experience to students within NL Health Services.
This position reports to the Regional Manager for Placement Services and maintains accountability to the Professional Practice Consultant for Social Work for professional integrity and practice issues.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador Association of Social Workers is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of community resources, systems and programs.
Knowledge of crisis intervention and supportive counseling.
Knowledge of case management.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to work effectively within an interprofessional environment and establish and maintain effective working relationships.
Demonstrated ability to work independently.
A satisfactory record of work performance and attendance is required.
