Newfoundland & Labrador Health Services (NLHS) is seeking a knowledgeable and resourceful Registered Nurse to join the Care Coordination Centre (C3) which is a provincial hub responsible for coordinating all interfacility transfers and repatriations across Newfoundland and Labrador as well as from external jurisdictions. Based at the Medical Communications Centre (MCC) at the Miller Centre site, this role is integral to ensure the timely, appropriate, and safe movement of patients both within the province and beyond its borders.

As a Registered Nurse at C3, you will manage real-time calls from physicians and patient representatives, assess clinical information, evaluate hospital capacity using specialized tools, and collaborate with healthcare and transportation partners to support effective and efficient patient transfers that promote health equity. This position is ideal for a nurse with extensive triage experience, strong clinical judgment, and a system-level understanding of patient flow.

Job Summary
Respond to transfer and repatriation requests from physicians and patient representatives across Newfoundland and Labrador and external jurisdictions.
Assess hospital capacity using tools such as Cognos and Capacity Planner.
Evaluate patient needs and anticipate necessary interventions or diagnostics.
Liaise with the Provincial Paramedicine and Medical Transport Program to confirm transportation availability and timelines.
Facilitate communication between referring and receiving physicians to confirm acceptance and finalize transfer plans.
Create and manage transfer requests in CAD in real time while effectively multitasking.
Coordinate with multiple stakeholders to ensure safe, timely, and appropriate patient transfers.
Prioritize cases using clinical judgment, triage skills, and patient flow principles in consultation with the Medical Transport Physician and Clinical ACP.
Contribute to quality assurance and audit processes.
Provide mentorship and clinical support to less experienced staff.

Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate Degree in Nursing.
Active practicing license with the College of Registered Nurses of Newfoundland and Labrador (CRNNL).
Minimum of five years (within the last seven) of nursing experience, with a strong background in Emergency and/or Critical Care and/or Medical Flight is required.
Proven expertise in clinical triage and critical decision-making in high-pressure environments.
Strong ability to multitask effectively while managing competing priorities in time-sensitive situations.
Experience with Meditech, CAD or other electronic health record systems is an asset.
Experience with communicating through interpretation services is an asset.
Knowledge of geography, access barriers, and health resources available across the province is an asset.
Demonstrated leadership in clinical coordination, health system planning, or emergency response is an asset.
National certification in emergency or critical care nursing (e.g., CEN, CCRN, CTRN) is an asset.
A satisfactory record of work performance and attendance is required.
