NL Health Services currently requires a Social Worker III for the Flexible Assertive Community Treatment Team, Mental Health and Addiction Services.
DUTIES: The FACT Team Lead functions as an administrative lead of the interdisciplinary team with duties including referral management, liaising with referral agents, chairing daily FACT meetings, coordinating the daily schedule, coordination of Psychiatry clinics and/or telehealth sessions, consultation and collaboration with internal and external services providers, coordination and monitoring of treatment plans and service delivery. This position demands a high level of independence with work assignments of a complex nature and the use of specialized skills in the treatment of mental illness and addictions within an interdisciplinary team.
The incumbent will also be responsible to provide clinical supervision and leadership to other team members and will assist with program planning and development, evaluation and change management.
While the primary function of this position is to support operations of the FACT Team, the Social Worker III may be involved in assessment, treatment, follow up and support of individuals with mental health and addictions needs throughout various mental health and addictions programs and services.
QUALIFICATIONS: The incumbent must possess advanced clinical knowledge and skills in the areas of assessment, intervention and case evaluation techniques in the area of mental health and addictions. The incumbent must have demonstrated leadership ability in the areas of interdisciplinary collaboration, advocacy, mentorship and program planning. The incumbent must have demonstrable skills in decision making, risk management and facilitation along with strong written and oral communication and organizational skills. This position demands specific knowledge and experience in the areas of mental illness, addictions and its impacts on individuals and families, program planning, team leadership, and coordinating professional services. The required knowledge and abilities for the position would normally be acquired through progressively responsible work in the area of mental health and addictions in an interdisciplinary service delivery model. Graduation with a Master?s Degree in Social Work is required, supplemented by at least (5) five years of related mental health and addictions experience. The incumbent must be eligible for registration with the Newfoundland and Labrador College of Social Workers. A satisfactory record of work performance and attendance is required.
