Position title: Registered Dietitian
- Complete admission resident nutritional assessment and determine level of nutrition risk as per Home nutrition profile/assessment and nutrition risk screening policy.
- Provide further nutritional assessment, documentation and development of nutritional care plans and follow-up for identified residents at high nutritional risk and some at moderate risk.
- · Participate in resident care planning regarding nutrition issues for new admission residents and those at high nutrition risk.
- · Calculate food intake records for identified residents eating poorly and make recommendations accordingly.
- · Review and approve changes made by the food service supervisor to the generic regular menu, therapeutic menu and nourishment menu for spring/summer and fall/winter cycle menus.
- · Customize individual menu plans for residents on specialized therapeutic diets, e.g.: renal diets.
- · Participate as a member of the swallowing assessment team, for identified residents with dysphagia and make recommendations accordingly.
- · Assist the Food Service Supervisor in interpreting and individualizing residents' modified and therapeutic diet orders.
- · Attend multi-disciplinary team conferences, for identified residents at high nutritional risk.
- · Maintain a dietitian's visitation list outlining the work completed (charts reviewed, etc.) and communicate diet/snack changes with the FSS.
- Must have current good standing with the College of Dietitians, as well as the Dietitians of Canada
- Must have a current vulnerable sector police clearance
