Definition of Work: This position will operate out of Deer Lake (Humber Valley White Bay Family Care Team), the incumbent will be providing services across multiple program areas including virtual care clinics. The Social Service Worker is responsible for the coordination and provision of non-professional social work services to meet client and family needs within the Family Care Team. Work involves supporting the role of Social Workers and other providers in the health care setting by coordinating client needs for concrete services; locating and arranging identified services through linking with a wide range of internal and external resources; consulting with members of the health care team and community resources and documenting interventions provided. The Social Services Worker will also assist clients and families with the completion of applications for supports to a variety of programs and services

The Social Services Worker is part of an interdisciplinary team responsible for Primary Health Care services based on the needs of the population in the health neighborhood. This can include the provision of services in a variety of settings and methods including in person visits, virtual care, and visits in clients homes including within personal care homes.

Required Qualifications: The successful candidate must hold at minimum: 2 year Community Studies-Family Services Diploma and/or a Bachelor?s Degree with a focus in the social sciences and a minimum of two years? work experience within a community-based services program is required. The incumbent must demonstrate strong written and oral communication skills. The ability to establish and maintain effective working relationships is essential. Experience working independently with limited supervision and a willingness to travel are also required. Mileage and appropriate per diems will be paid in accordance with the CUPE Collective agreement.

**A satisfactory record of work performance and attendance is required for this position.
