Definition of Work:
This position will report to the Director of Primary Health Care and Rural Health and will focus on the health of the population advancing primary health care and chronic disease frameworks. Responsibilities include:
? Collaboration with community stakeholders and health care professionals to coordinate, plan, implement and evaluate primary health care & chronic disease prevention/management (CDPM) strategies.
? Collaboration with Family Care Teams in the planning, coordination, and implementation of program initiatives that align with Health Accord NL and the Family Care Team Policy Framework.
? Coordinating the implementation and evaluation of the Primary Health Care Framework, Family Care Team Policy Framework and expanded Chronic Disease Models within NLHS.
? Provide strategic leadership and oversight to the Primary Health Care Facilitators within the Central Zone, ensuring their work is aligned with the transformational goals of the NL Health Accord and the strategic priorities of NL Health Services.
? Ensuring a standardized approach to the prevention and management of chronic disease across the region while continuing to enhance programs (e.g. Telehealth, Better Program, Heart Failure Outreach).
? Partnering with Family Care Teams for the provision of primary health care & chronic disease prevention/management education and consultative services to health professionals, educators and peer leaders.
? Participation in internal and external committees to plan primary health care & chronic disease prevention/management initiatives, develop resources and provide related expertise.
? Participation in the planning and implementing of evaluation/research projects and service area grants related to the prevention/management of chronic disease.
? Provide effective leadership for program aspects of diabetes services across the region and manage the day to day operational aspects of diabetes services at JPMHRC and the CNRHC.
? Ensure a best practice approach to program/policy development and service delivery.

Required Qualifications:
? Bachelor?s degree in a health-related discipline.
? Knowledge and experience in primary health care & chronic disease management, health program planning, implementation and evaluation; including community mobilization, health education, etc. is essential.
? Strong written and oral communication skills, facilitation skills, leadership ability and knowledge of adult learning principles are essential.
? Previous experience in program leadership in the area of primary health care & chronic disease prevention and management would be considered an asset.
? A significant degree of independence, judgement and initiative is required.
? Consistent demonstration of NL Health Services corporate values; Innovation, Compassion, Accountability, Respect and Excellence (I CARE)
? A satisfactory record of work performance and attendance is required for this position.
? A satisfactory certificate of conduct from the RCMP or local police authority.
