Job Summary

This Patient Flow manager role will provide a collaborative leadership role, working with clinical leadership, program managers and Directors across multiple sites and programs. In this position, you will manage patient flow, monitoring appropriateness of admission and patient placement and coordinate resource activities to ensure the appropriate resources are available to meet patient care requirements for timely home discharge and access to care. This role is an integral part of the Integrated Operations Center team, located at Carbonear General Hospital.

The Manager role will primarily work days and some evenings to execute and lead the improvement goals of the zone, and strategic priorities as they relate to enhanced access and flow. This position will be responsible and accountable for the development and overall coordinating of clinical activities related to patient flow within the zone acute care facilities and with clinical programs in the region. This will include but is not limited to, facilitation of team/unit/program-based huddles, monitoring and measuring inputs and outcomes associated with EDD, ALC, LOS and other quality metrics associated with access and flow. This includes the facilitation of visual managements systems, dashboards and other monitoring/intake systems developed by the programs, including ICM that are associated with access and efficiency across the care continuum. The role will work with staff and managers with the goal of ensuring effective patient flow, and facilitation of seamless transitions between episodes of care. This role functions in a leadership capacity ensuring appropriateness of admission, length of stay and repatriation, using clinical best practice guidelines and benchmarking. The Manager will also liaise with regional managers and staff in acute care, community supports, and long-term care to facilitate timely access to appropriate care in accordance with the standards developed in partnership with the Provincial Clinical Efficiency program and the recommendations from the Provincial ALC working group.

Job Qualifications

EducationA baccalaureate degree in a health-related discipline is required.
A master?s degree is preferred.

ExperienceA Minimum of five years recent acute care clinical experience required. Experience with patient flow would be an asset. Recent management and quality improvement experience an asset.

Competencies (Knowledge, Skills & Abilities)You demonstrate knowledge of quality improvement principles, human resource management, utilization management principles and discharge planning as well as the ability to determine the clinical needs of patients.
