The Senior Coordinator, CHW Programs supports the delivery of the Transportation and Gathering Centre programs. The role actively contributes to planning activities for Elders, address social isolation, coordinate with other programs and partners to support Elders' goal of living well and safely in their home, as well as identify any service gaps. The role also supports the program delivery by recommending improvements.

The Senior Coordinator, CHW Programs includes planning activities for Elders to address social isolation, coordinating with other programs and partners to support Elder's goal of living well and safely in their home.

In this role, you will:

  • Assess client needs on eligibility to access the program(s) according to established policies and procedures.
  • Onboarding new clients for program by reviewing program offerings and parameters. Suggest and create a care plan to meet their needs.
  • Liaise with community partners to determine vulnerability and gaps in program/services.
  • Evaluates service delivery, and makes recommendations for program improvement.
  • Provides on-site support to management of physical assets (premises, vehicles, security).

What we are looking for:

  • Minimum of 3 years of experience and a 1-2-year post-secondary certification/diploma in health related field, gerontology or social services, or less experience if combined with a 3-year college diploma.
  • Experience and knowledge related to Indigenous communities is a requirement.
  • Ability to participate in food preparation.
  • Ability to travel throughout the Moosonee community and must have a valid driver's license.
  • Strong written and verbal communication in English is required; working knowledge of Cree is a strong asset.
  • Ability to solve complex problems and adapt to changing circumstances in a fast-paced environment.
  • Sound knowledge of various computer programs such as MS Office (Outlook, Word, Excel, PowerPoint).
  • Experience with data software assessments is an asset.
  • Strong communication, and customer service skills.

Working Conditions:

  • This position is based out of the Moosonee office location which is in a remote northern fly in community area
  • The incumbent must have the ability to travel throughout the Moosonee community and Northeast Region
