Definition of Work:
This position will function as a member of the Community Support team working with community health nurses, social workers, coordinators and community partners. Responsibilities include palliative care, health assessment, wound care and health maintenance / promotion in consultation and collaboration with the Registered Nurse. The Licensed Practical Nurse I (LPNI) working with the Continuing Care Nurse I performs professional community health nursing work carrying out community health programs in homes, schools, clinics, and other community settings. The successful candidate provides nursing care, assists in the assessment of community health needs, undertakes case management, and conducts school health programs and the provincial immunization program in consultation and collaboration with the Registered Nurse. Applicants must be competent to work with-in a full scope of practice including health assessment, venipuncture, intravenous therapy, catheter care, wound assessment and therapy. The applicants will be re-quired to demonstrate these skills during an observed clinical exam. Other duties within the LPN scope of practice will be required. The LPN I is involved in health promotion activities, engages in community development, facilitates group sessions, conducts health education sessions in schools and communities.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centred care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Physical Demands:

• Constantly moves/lifts up to 10-15lbs, regularly moves/lifts up to 50lbs; re-quired to restrain/reposition/move/lift up to, or above, 50lbs with co-worker assistance and repetitively required to complete pushing/pulling with medium force (22-44lbs).
• Constantly stands/walks. May work in awkward or cramped positions to as-sist with care. Work repetitively involves reaching/stretching, bending/twisting and repeated use of low level postures such as squatting, crouching or stooping.
• Potential for exposure to blood, bodily fluids and/or other contagion. Potential to be exposed to patients/residents with potential risk for aggressive behavior.
• Work environments present potential for exposure to scented/chemical products and/or other allergens within publicly accessible areas.

Required qualifications:
• Graduation from a practical nursing program approved by Central Health.
• Current Basic Life Support (BLS) certification.
• Current registration with the College of Licensed Practical Nurses of Newfoundland and Labrador. Must be able to work at full scope of practice and demonstrate these skill sets.
• Two (2) years clinical experience.
• Demonstrated proficiency in Wound Care Management and Medication Administration including intramuscular, intravenous, and subcutaneous injections.
• Willingness to complete advanced education in:
o Administration of immunizations.
o CADD pump medication administration.
o Advanced wound care including packing and negative pressure therapy.
o Other additional skill modules as available.
• Demonstrated effective interpersonal and communication skills. Presentation with a PowerPoint format (provided) will be a required component of the interview for this position.
• Demonstrated ability to work independently while effectively working with
support of the Community Supports team.
• Willing to work a flexible work schedule.
• The use of a Private Vehicle is a condition of employment.
• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory Certificate of Conduct from the RCMP or local police authority.
