About the Role
Labrador Grenfell Health requires an experienced Resident Care Facilitator to provide day-to-day assistance and support to the Clinical Nurse Manager at a 70-bed long term care home in Happy Valley Goose Bay. The ideal candidate is an experienced Registered Nurse with excellent leadership, communication and organizational skills with experience in quality improvement and risk management initiatives to improve optimal delivery of resident care. The position is accountable to the Resident Care Manager.
Qualifications:
- Graduation from an approved/recognized school of nursing program is required;
- Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
- Minimum of 3 years of clinical experience preferably in long term care;
- Breastfeeding course, or willingness to complete
- Current BCLS Certification;
- Proficient use of Microsoft, Meditech, MDS 2.0 and MS Outlook;
- Good interpersonal, communication, organizational, time management and conflict resolution skills;
- The successful applicant will be required to submit a satisfactory certificate of conduct;
Duties: (Included but not limited to)
- In collaboration and consultation with the Resident Care Manager plans and organizes resident care;
- Facilitates, organizes assists to lead variety of initiatives;
- Monitors and measures quality improvement initiative;
- Provides day-to-day direction to LPNs and PCAs including daily resident assignments;
- Acts as a Lead for the clinical long- term care programs;
- Functions as a liaison to other health care providers, internally and externally;
- Maintains ongoing communication with families regarding resident care;
- Participates in quality improvement activities as required;
- Participates in committees as required;
- Completes Occurrence investigations as per set protocols;
- Monitors and evaluates staff performance;
- Submits reports to the Resident Care Manager as indicated;
- Other duties as assigned;
