Job Summary:

The interRAI Community Health Assessment (CHA) Master Trainer provides training and support for all South West Home & Community Care Community assessors, while supporting clients with comprehensive assessments and ensuring a high-quality client experience, while regularly completing responsibilities of the client coordinator role. The interRAI CHA Master Trainer ensures the completion of client assessments, creation of care plans and referrals to local programs/services is within the framework of VON policies, and Community Support Service and Development Quality Committee as outlined in the Memorandum of Understand for the LHIN.
Key Responsibilities:

  • Train/re-train Client Service Coordinators within South West VON and other organizations within the South West Home & Community Care collaborative as directed by the Community Support Services (CSS) Memorandum of Understanding (MOU)
  • Evaluate new Client Service Coordinators for competency and report results to their manager
  • Train South West VON Client Service Coordinators, South West Home & Community Care collaborative, and external organizations on AUA (Assessment Urgency Algorithm) screener tool
  • Support and guide VON CHA Client Service Coordinators in the South West Home & Community Care collaborative for interRAI CHA issues and technical support
  • VON point of contact for Central Scheduling for Inter RAI CHA issues, and data entry statistical questions
  • Participate in regular Master Trainer meetings with all South West LHIN collaborative Master Trainers
  • System on boarding and off boarding of interRAI CHA users including signing authority for One Care Total Health Care (THC) for employee THC and One Care Portal access and maintaining these records
  • Ensures removal of access to maintain privacy with the THC if staff is not accessing the system
  • Set up and lead team meetings/teleconferences with SW Assessors
  • Completing and maintaining active lists of SW CHA Client Service Coordinators who are utilizing the interRAI tools
  • Supports and maintains client caseload in accordance with program criteria, guidelines and directives
  • Maintains competency including the ability to interpret information in interRAI CHA & CAPs/care planning
  • Completes client assessments in the home using conversation-based methodology
  • Develops and conducts a comprehensive therapeutic rapport
  • Utilizes technology in the home to complete comprehensive assessments
  • Identifies and prioritizes environmental risk for client's health and safety such as shelter, food and running water, temperature control and overall condition of living
  • Develops care plan/referral plan (interpreting the CAPs and care planning)
  • Sends referrals for services identified by Client Service Coordinators to recipient agencies and tracks/receives confirmation of receipt of referrals
  • Accesses and reviews client assessments via the IAR (Integrated Assessment Record)
  • Collects and reports assessment statistics and relevant data to Manager
  • Adheres to the prescribed privacy policies and standards for Community Support Service agencies including VON
  • Positively and professionally represents all CSS agencies and service providers
    Common Responsibilities
  • Promotes the goals and values of VON and their role as an integrated community care provider
  • Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations.
  • Abides by all VON policies and work practices
  • Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place within their role
  • Works in collaboration with other staff in a team approach to service delivery

External and Internal Relationships:

  • Maintain strong partnerships with Master Trainer Community Support Agencies within the South West LHIN as directed through the MOU
  • Consults with referring agencies on care plans to ensure shared understanding of client needs and appropriate interventions/services to meet those needs
  • Engages frequently with Scheduler, Community Support Coordinator and Manager
  • Networks and collaborates with regional partners and agencies

Education, Designations and Experience:

  • University Degree or College Diploma in Heath or Social Services field
  • Minimum of 3 years related experience including Coordinator level experience in a home and community care organization or social services program
  • Broad working knowledge of community resources, including service and programs within the defined catchment area.
  • Successful completion of interRAI CHA and Momentum software training
  • Successful completion of Community Care Information Management (CCIM) Master Trainer training and Momentum Super User training

Skill Requirements:

  • Strong attention to detail and accuracy
  • Skilled in communication (verbal, written, listening), rapport building, and relationship management
  • The ability to use analytical skills and understand/interpret client assessment data
  • Ability to effectively manage multiple tasks, while prioritizing, meeting deadlines, and working well under pressure
  • Demonstrated skills in leadership, team building, communication, and education
  • Proven experience in building external and community relationships
  • Time management and strong organizational skills
  • Ability to work independently and as part of a team
  • Ability to accept and facilitate change
  • Ability to maintain confidentiality
  • Knowledge and skills in comprehensive assessment and the ability to maintain proficiency
  • Computer proficiency in Windows OS and Microsoft Office programs, Momentum, IAR (Integrated Assessment Record), THC (Total Health Care)
  • Experience working with volunteers and seniors is an asset
  • Must be able to wear Personal Protective Equipment (PPE)

  • Available for weekend assessment coverage
  • Must have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
  • A current and original copy of a satisfactory Criminal Records Check
  • A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required
  • The use of Personal Protective Equipment (PPE) may be required
