Home Health Aide

People stay safer at home when daily care is skilled, consistent, and human. In this role you turn a clinical plan into real life—protecting dignity, preserving independence, and catching change before it becomes a crisis. Families and nurses rely on you between visits; the quality of those hours decides whether someone remains comfortable at home.

Performance Objectives
  • Complete personal care and activities of daily living so each client meets the hygiene, nutrition, mobility, and comfort goals in the care plan.
  • Spot and report changes in condition early enough for the clinical team to act before a preventable decline or hospital return.
  • Carry out safe transfers, ambulation, and fall-prevention routines within assessed limits, with no unreported incidents.
  • Prepare meals and give authorized medication reminders exactly as directed, documenting every deviation the same day.
  • Keep visit notes complete and on time so supervisors and families can see what was done and what changed.
  • Sustain reliable client relationships that protect continuity and reduce missed or shortened visits.
Environment & Resources

You work in clients' homes on community and longer-term care pathways. Registered nurse placement managers match assignments to your skills and set the care plan, schedule, and escalation path before you arrive. You report to a clinical supervisor, with a nurse available for clinical questions. Orientation, supplies guidance, and skills refreshers are provided. You bring reliable transportation and a phone for same-day documentation.

Essential Qualifications
  • Recognized Home Health Aide, Personal Support Worker, or equivalent certificate, plus current CPR and first aid.
  • Hands-on personal care experience with older adults or people living with chronic illness in a home or care setting.
  • Proven ability to follow a written care plan, recognize red flags, and escalate without delay.
  • Clear, respectful communication with clients, families, and clinical staff.
  • Dependable attendance and accurate same-day documentation.
  • Work eligibility and the background and health clearances required for home care.
Why This Role
  • Nurse-led matching, not a generic dispatch queue.
  • Pathways across community, long-term, and post-acute recovery as your skills grow.
  • Colleagues held to the same standard of care you are expected to deliver.
  • Clinical backup so you are not left guessing in the home.

If a good day is a client who is safer, fed, clean, and respected, apply now. Show us where you have delivered that standard, and we will match you to the homes that need it.
