Right at Home Canada Fraser Valley is a locally owned home-care company serving older adults and adults with disabilities in Abbotsford, Chilliwack, Mission, Langley and surrounding communities. We help clients live safely and comfortably at home through personalized care and practical support for their families.
We are looking for a Case Manager who will be the primary architect of the client care journey. You will coordinate resources, design care teams, align with clinical and operational standards, and serve as the key point of contact for clients, families and caregivers. You'll also partner with business development and community relations efforts, helping to grow our service footprint in the region.
What you will do
· Visit clients at home, learn about their needs and routines, and write clear care plans. Update plans when their needs change.
· Help choose caregivers whose skills, experience and availability suit each client.
· Show caregivers how to carry out care tasks, watch them practise and follow up so they are ready for their assignments.
· Check in with clients, families and caregivers.
· Address concerns, keep records current and seek further help when needed, within your LPN scope of practice.
· Help the office team set daily priorities, follow up on tasks and arrange coverage when schedules change.
· Review weekly care hours, cancelled visits and shifts needing coverage.
· Work with the owner to improve coverage and identify when more caregivers are needed.
· Help interview and welcome new caregivers. Respond to service inquiries and help new clients get started.
· Build connections with local health and community contacts.
· Take part in community events and explain our services to individuals or small groups.
· Give the owner a short weekly update on clients, staffing, care hours and outstanding follow-ups.
What you need
· Current practising LPN registration with BCCNM or equivalent, and experience caring for older adults.
· Good organization, clear communication and reliable follow-through.
· You should be comfortable giving caregivers practical guidance and taking initiative.
· Comfort using email, Microsoft Office and electronic care records.
· A valid driver's licence, a reliable vehicle, and a current criminal record check.
