Dibac Home Support is a licensed Temporary Help Agency providing personal care, household support, postpartum care, and in-home childcare to families and facilities across the Kitchener-Waterloo-Cambridge region. THE ROLE
The Care Coordinator is one of the most important roles at Dibac. You are the person who makes sure every client has a worker, every worker knows where to be, and every family feels supported. This is a scheduling and coordination role at its core — but what makes it great is the relationships you build with clients, families, and workers over time. If you are calm under pressure, genuinely care about people, and have a talent for keeping complicated schedules organized, we want to hear from you.
WHAT YOU WILL DO
· Build and manage the weekly schedule for all active clients across all four Dibac service lines
· Respond to call-outs and find replacement workers promptly — communicating changes to clients and families with care
· Serve as the primary contact for clients, families, and workers for day-to-day questions, concerns, and changes
· Complete and provide Assignment Employee Information Sheets (AEIS) to workers before each new assignment
· Conduct client intake calls and gather information using Dibac's intake forms
· Monitor visit documentation and follow up on missing or incomplete shift notes
· Escalate service quality concerns and worker conduct issues to the Director
· Maintain accurate records in Dibac's scheduling and client management system
· Support after-hours coverage responsibilities as part of a rotating or shared arrangement
WHAT WE ARE LOOKING FOR
· Minimum 2 years in a scheduling, coordination, or client-facing role — healthcare or home care preferred
· Excellent organizational skills — manages competing priorities and time-sensitive changes with composure
· Warm, professional communication skills — comfortable with difficult conversations
· Calm under pressure — shift changes and urgent coverage needs are part of the job
· Proficiency in scheduling tools and Microsoft Office
· Discretion with confidential client and employee information
· Second language an asset
· Legally eligible to work in Canada
