The Clinical Care Coordinator provides clinical, compliance, and quality improvement support across multiple retirement residences throughout Eastern Ontario. Working closely with the Regional Health Care Director, this role plays a key part in supporting clinical oversight, regulatory compliance, inspection readiness, and the development of standardized clinical systems and processes across the organization.
This position offers the opportunity to contribute to organizational growth while supporting excellence in resident care, quality improvement initiatives, and ongoing compliance across multiple residences.

Key Responsibilities (including, but not limited to):
- Support multiple retirement residences across Eastern Ontario with clinical, compliance, and quality improvement initiatives.
- Work directly with the Regional Health Care Director to support clinical oversight, regulatory compliance, inspection readiness, and operational excellence across the organization.
- Complete and review resident assessments, care plans, and clinical documentation to ensure quality resident care and regulatory compliance.
- Conduct clinical, medication, and compliance audits and support follow-up on identified concerns.
- Assist in identifying clinical risks, compliance concerns, audit findings, trends, and opportunities for improvement across residences.
- Support residences with inspection preparation, corrective action plans, and ongoing compliance monitoring.
- Participate in quality improvement, audit, and regulatory compliance initiatives designed to enhance resident care outcomes and organizational performance.
- Support the development, implementation, and continuous improvement of standardized clinical and compliance policies, processes, tools, and systems across the organization.
- Assist with medication management practices, pharmacy-related requirements, infection prevention measures, outbreak management, and associated documentation.
- Monitor staff training requirements, certifications, and clinical competencies to ensure ongoing compliance.
- Provide education, coaching, and guidance to residence leadership teams and clinical staff.
- Conduct regular site visits to support resident care, quality improvement activities, and compliance initiatives.
- Report clinical trends, audit findings, risks, and compliance concerns to senior leadership and recommend corrective actions where appropriate.
- Contribute to organizational growth initiatives and the ongoing enhancement of clinical systems, processes, and best practices.
Qualifications
- Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
- Experience in retirement homes, long-term care, assisted living, or community healthcare is preferred.
- Knowledge of resident assessments, care planning, medication management, quality improvement, and regulatory compliance.
- Experience conducting audits, supporting inspections, and implementing corrective action plans is considered an asset.
- Excellent organizational, communication, leadership, and problem-solving skills.
- Ability to work independently while managing competing priorities across multiple locations.
- Valid driver's license and ability to travel extensively throughout Eastern Ontario. Company vehicle will be provided.
