As the Adult Day Program Coordinator, you will play a key role in delivering high-quality client care within the Adult Day Program, including administering medications, supporting clients with care needs, and providing guidance and mentorship to Recreation Aides.
Responsibilities
- Develop and update individualized care plans under the direction of the Director of Care or designate, including assessment of current and future client needs.
- Evaluate the effectiveness of care and report significant changes to the Director of Care, AHS Case Manager, and family as appropriate.
- Provide personal care assistance, medication administration, nursing procedures, and client education.
- Lead, support, and mentor Recreation Aides in delivering high-quality client care and following appropriate procedures.
- Coordinate and implement daily programs and engage clients through meaningful interaction.
- Observe, document, and report on client care, services, and overall well-being.
- Communicate client concerns to physicians or management as needed.
- Manage recreation equipment, including inventory and basic maintenance.
- Set up and clean up for events and activities.
- Demonstrate flexibility to meet changing client needs and apply evidence-based practice in caring for aging adults.
- Ensure compliance with provincial continuing care standards, legislation, and Occupational Health and Safety requirements.
- Promote safe work practices and accident/injury prevention
Qualifications and Experience
- Completion of an Allied Health or LPN diploma.
- Active registration and good standing with respective regulatory body or association.
- Experience in an Adult Day Program setting is preferred.
- Demonstrated knowledge of infection control practices.
- Exceptional interpersonal and communication skills.
- Strong organizational skills with the ability to handle multiple client requests.
- Proficiency with Microsoft Office and general computer applications.
- Strong ability to prioritize, problem-solve, and work independently.
- Ability to stand for extended periods and safely bend, squat, kneel, and lift up to 25 lbs.
- Demonstrated compassion, professionalism, and skill when assisting clients
