Their West Park long-term care home in Toronto are committed to improving the quality of life for their residents through compassionate, innovative, and collaborative care. They are currently seeking a dedicated and experienced Nurse Practitioner to join their team as an independent contractor and lead clinical care and research initiatives within their dynamic and resident-focused environment.

Position Summary:

The Nurse Practitioner (NP) will play a critical role in delivering comprehensive healthcare services to residents of their long-term care home. This includes direct resident care, therapeutic management, health promotion, and participation in research and education activities. The ideal candidate will be a passionate, personable, and research-oriented professional who can thrive in a complex care environment.

Key Responsibilities:

  • Deliver primary healthcare to residents including the management of acute and chronic

conditions, preventative care, and therapeutic interventions.

  • Conduct health assessments using comprehensive tools and clinical reasoning.
  • Lead or support key resident care areas: admission/discharge planning, hospital transitions, dementia care, pain and palliative care, and responsive behaviour management.
  • Interpret diagnostic results and engage residents and families in developing personalized care plans.
  • Collaborate closely with interdisciplinary teams, physicians, residents, and families to ensure continuity of care.
  • Participate in and lead education, quality improvement, and research initiatives in partnership with organizations like Gamage and Humber.
  • Mentor and supervise clinical learners including nursing/medical students.
  • Work effectively across all stakeholder groups and maintain positive team dynamics.
  • Engage in after-hours and on-call support, as needed.

Qualifications:

  • Registration with the College of Nurses of Ontario (CNO) in the Extended Class.
  • BScN required; MScN or MN is considered an asset.
  • Membership in the Nurse Practitioners' Association of Ontario (NPAO) preferred.
  • Current professional liability insurance (e.g., through RNAO or equivalent).
  • National certification in gerontology is an asset.
  • 3+ years of experience working as an NP in Canada is strongly preferred.
  • Previous experience in long-term care, geriatrics, or dementia care is an asset.
  • Strong communication, leadership, and interpersonal skills.
  • Demonstrated commitment to research, education, and quality improvement.
  • Flexible availability for on-call, evenings, or weekends.
