We are hiring a Case Manager for our Case Management Team.
Role: Contract, Full-Time: Until March 27, 2026. 35 hours per week, evenings and weekends may be required.
Closing date: September 5, 2025
Salary: Starting salary range $31.01 - $33.22/hour. *This represents the first three steps in a seven-step salary grid. Candidate experience will be considered when determining the starting salary.

Location: The successful candidate will work primarily from our Alliston Office, located at 35 Victoria Street East, Unit 1, Alliston. Some travel to other CMHA YRSS locations will be required within York Region & South Simcoe.
Job Profile:
The Case Manager supports individuals aged 16+ experiencing serious and persistent mental illness impacting daily functioning. Working collaboratively with clients, the Case Manager helps identify strengths, develop goals, and connect to services and supports that promote recovery, independence, and improved quality of life. Services are individualized, flexible, and recovery-focused, aligning with empowerment principles and the Intensive Case Management Standards (2012).
Key Responsibilities:
Direct Client Service:
- Maintain a client caseload, including extensive travel in rural areas
- Perform comprehensive assessments and develop individualized case management plans
- Establish collaborative, client-directed relationships using empowerment and recovery principles
- Support goal development, symptom management, life skills, and community integration
- Provide referrals, advocacy, counseling, and crisis planning
- Link clients to formal and informal community resources and supports
- Attend case conferences and health-related appointments
- Monitor progress and adjust plans as needed
- Support discharge planning and transition to appropriate services
- Facilitate groups and promote client participation in community activities
- Provide education on mental health, recovery, and available resources
- Support clients in wellness planning (diet, exercise, self-care)
- Respond appropriately to trauma and relapse using strength-based interventions
Documentation:
- Maintain timely, accurate, and comprehensive client documentation in CRMS and Outlook
- Complete intake assessments, case notes, progress summaries, and discharge plans
- Complete paperwork to support clients at risk (e.g., eviction)
- Use data systems to ensure accuracy and accountability
Advocacy & System Navigation:
- Advocate for client access to mental health and community services
- Identify and act on service gaps and systemic barriers
- Support clients in self-advocacy and autonomous decision-making
- Engage in individual and systemic advocacy with support from leadership
Community Engagement & Liaison:
- Collaborate with hospitals, schools, justice system, and community agencies
- Educate the public, agencies, and institutions on mental health and referrals
- Support community capacity-building and engagement initiatives
- Represent CMHA at community events or committees, as approved
Organizational Participation:
- Engage in supervision, staff meetings, and professional development
- Participate in staff selection processes and internal initiatives
- Promote and uphold CMHA's mission, values, and policies
- Contribute to program and agency-wide safety practices and quality improvement
Client Safety:
- Follow program guidelines and client safety frameworks
- Collaborate with clients on prevention and safety planning
- Support response to health and safety concerns in accordance with policy
Student Supervision (as applicable):
- Supervise field placement students and assist with onboarding
- Complete college/university documentation as required
Knowledge & Skills:
- Knowledge of empowerment and recovery-based case management
- Familiarity with the Mental Health Act, Substitute Decisions Act, Health Care Consent Act, and PHIPA
- Strong understanding of mental health, systemic issues (e.g. poverty, stigma), and local services
- Excellent oral, written, and interpersonal communication skills
- Proficient in Microsoft Office and Outlook
- Creative problem-solving and conflict resolution abilities
- Ability to assess functional abilities and mental status
- Experience working collaboratively with diverse teams and clients
Additional Requirements:
- Valid Canadian driver's license and $1,000,000 liability insurance (including client transport)
- Satisfactory Vulnerable Sector Screening
- Ability to travel extensively in the region
- A second language reflecting the local community is an asset
