Detailed Overview
Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:
Reporting to the Aboriginal Health Manager East, the Coordinator develops, plans, facilitates and coordinates the program to meet identified needs of the community it serves. The Coordinator collaborates with other programs across Fraser Health as well as community partners to ensure the development and implementation of the program is aligned with community needs.
Responsibilities
- Designs, develops, plans, facilitates and coordinates the program for both virtual and physical programming for older adults.
- Facilitates community virtual and in-person presentations and workshops.
- Organizes workflow; develops, coordinates and implements work plans that support the objectives of the program; determines priorities; resolves day-to-day problems; recommends process changes for workflow and staffing levels.
- Participates in the selection, hiring, and orientation of the program team members; identifies development and training opportunities for staff.
- Collaborates with FH departments, FNHA and community partners to develop, implement, evaluate, and update program specific goals and objectives, and related guidelines.
- Engages with community partners to ensure program is developed and implemented inline with community needs and includes cultural content.
- Allocates resources and coordinates team scheduling of the program to ensure equitable service delivery.
- Tracks and maintains record of program activities; develops, compiles, and analyzes reports that help inform quality improvement recommendations for program.
- Assists with monitoring expenditures within the established parameters; provides recommendations regarding budgeting priorities for the program.
- Acts as a resource to care teams and provides consultative advice to external and internal stakeholders as required on all matters related to the program.
- Coordinates and participates in client case conferences to ensure the clients? needs are met.
- Establishes and maintains an effective working relationship and ongoing communications with other departments, community agencies and key stakeholders.
- Attends and participates on various committees and working groups with respect to the program.
- Performs other related duties as assigned.
Qualifications
Education and Experience
Bachelor's degree in Health or Social Sciences, Business or related field. Three (3) to five (5) years related experience working with Elders and Indigenous community health. Knowledge of Indigenous cultures, experience facilitating groups and program development. Or an equivalent combination of education, training and experience.

- Ability to work effectively under pressure and with changing priorities and deadlines.
- Proficiency in the use of a personal computer and applicable office software applications as effective tools to achieve results.
- Physical ability to carry out the duties of the position.
