Job Details:
Key Responsibilities:
- Conduct comprehensive clinical assessments, diagnose, and treat a broad spectrum of acute and chronic conditions.
- Prescribe treatments, order diagnostic tests, and monitor patient progress.
- Coordinate care and manage follow-up to ensure high-quality, continuous patient management.
- Provide culturally sensitive care and education to patients and their families, emphasizing the connectedness of emotional, spiritual, physical, and mental well-being.
- Work within a multidisciplinary team comprising Physicians, Nurse Practitioners, R.N. (E.C.)’s, a Diabetes Care Team, Mental Wellness and Addiction Supports, Traditional Healers, Elders, and community health supports in Advocacy, Health Promotion, and Education.
- Develop collaborative practices with team members and accept referrals and consultations from Nurse Practitioners, R.N. (E.C.)’s, and other allied health professionals.
- Maintain accurate and timely medical records in compliance with regulatory and institutional policies.
- Contribute to team meetings, quality improvement initiatives, and community outreach programs.
- Assist in policy development and planning of health promotion activities that integrate traditional healing methods and Aboriginal cultural values.
Education & Certification Requirements:
- Medical degree (MD or DO) from an accredited institution.
- Board certification or eligibility in Family Medicine, with a valid C.C.F.P. certificate.
- Must hold a Certificate of Good Standing from the College of Physicians and Surgeons of Ontario.
- Licensed to practice by the Ontario College of Physicians and Surgeons.
Experience & Skills Requirements:
- Proven clinical experience, preferably with a background in Indigenous or community-based care.
- Experience in Aboriginal communities is an asset.
- Excellent interpersonal and communication skills.
- Demonstrated ability to work effectively within a multidisciplinary team.
- Proficiency with electronic health record (EHR) systems and standard office software.
- Commitment to culturally sensitive, community-based healthcare.
