LifeHouse Case Worker

The Case Worker for the will provide vital support to residents in our permanent housing, working closely with individuals and families to help them achieve stability, self-sufficiency, and personal goals. The Case Worker will also engage with community resources and landlords to ensure residents have access to the necessary services and housing opportunities. This role involves individualized case management, community engagement, and advocacy.

This is a one year contract with the possibility of extension.

Key Position Objectives:
Resident Support:

  • Provide one-on-one support to residents, assessing individual needs and developing case plans that promote self-sufficiency, mental health, and housing stability.
  • Assist residents with accessing resources such as financial assistance, healthcare, employment services, mental health support, and community programs.
  • Offer advocacy and guidance for residents experiencing barriers such as substance abuse, mental health challenges, or legal issues.
  • Facilitate workshops or group sessions on life skills, budgeting, tenant rights, and other relevant topics.
  • Monitor and follow up on residents' progress towards achieving their goals, adjusting case plans as necessary.
  • Work with tenants to foster a positive community environment within the housing units, promoting cooperation, respect, and support among residents.

Community Engagement:

  • Build and maintain relationships with local service providers, organizations, and community members to connect residents with external supports such as employment, education, and healthcare services.
  • Act as a liaison between residents and various community partners to create individualized support networks.
  • Coordinate outreach efforts to increase awareness of available services and housing opportunities in the community.
  • Landlord Engagement:
  • Develop and maintain positive relationships with landlords and property managers, ensuring effective communication and smooth coordination of housing placements for residents.
  • Mediate conflicts between landlords and tenants when necessary, advocating for fair treatment and tenancy rights for residents.
  • Provide ongoing support to residents and landlords to promote successful tenancies and reduce the risk of eviction.

Administrative Duties:

  • Maintain accurate and up-to-date case files, documenting interactions, support services provided, and progress made by residents.
  • Prepare reports as required by program management, detailing housing updates, community partnerships, and resident outcomes.
  • Assist in gathering data for program evaluations and funding applications.
  • Ensure compliance with all organizational policies, procedures, and relevant legislation.

Qualifications:

  • Post-secondary degree or diploma in social work, community development, or a related field, or equivalent work experience.
  • Experience in outreach, case management, or social services, particularly in transitional housing or homelessness prevention programs.
  • Strong knowledge of local community resources, housing systems, and tenant rights.
  • Excellent communication and interpersonal skills, with the ability to work with diverse populations and navigate complex social issues.
  • Conflict resolution and problem-solving abilities.
  • Ability to work independently and as part of a team.
  • Valid driver's license and access to reliable transportation is preferred.
