Summary

We are seeking a Care Navigator to join our team at Forbes Pharmacy and Blue Sky Professional Services. As a Care Navigator, you will serve as both a care provider and a team lead. When appropriate, you'll assess client needs, review care documentation, conduct quality control visits, and step in to cover care shifts when needed. This role requires flexibility, a warm and encouraging presence, and strong leadership skills.

Key Responsibilities (this is an overview and not an exhaustive list):

Provide in-home care to clients when needed
Prepare written communications and administration tasks
Conduct regular quality control visits and assessments
Review communication binders and provide feedback
Mentor and support other caregivers
Step in to cover urgent or last-minute care shifts
Coordinate care in collaboration with our leadership team and may include clients, families, and team members

What We're Looking For:

Experience as a Care Aide or Internationally Trained Nurse
Demonstrated reliability and professionalism
Strong interpersonal and problem-solving skills
Leadership qualities and sound judgment
A valid driver's license is required for the position (reliable vehicle preferred)

Pay: From $48,750.00 per year
