Location: Flexible within New Brunswick, with preference given to Moncton or Saint John

THE OPPORTUNITY:

The Heart and Stroke Foundation of New Brunswick is seeking a compassionate, motivated, and community-focused Stroke Navigator to join our Health Promotion team.

Funded by the Government of New Brunswick, the Stroke Navigation Program helps individuals and families successfully transition from hospital to home following a stroke. As a Stroke Navigator, you will provide personalized, non-clinical support to help clients navigate the healthcare system, connect with community resources, overcome barriers, and build confidence throughout their recovery journey.

Using health promotion principles, education, and individualized guidance, you will empower clients to improve their quality of life while supporting the community reintegration pillar of the Canadian Stroke Best Practices Continuum of Care. (https://www.strokebestpractices.ca/).

This role is ideal for someone who enjoys building relationships, collaborating with healthcare community partners, and making a tangible impact every day.

KEY RESPONSIBILITIES:

  • Provide individualized stroke navigation services that support recovery, continuity of care, and improved quality of life.
  • Conduct needs assessments and connect clients with appropriate health, rehabilitation, and community resources.
  • Serve as a trusted resource for clients, caregivers, healthcare providers, and members of the public.
  • Promote the Stroke Navigation Program through community outreach, presentations, and stakeholder engagement.
  • Build and maintain strong relationships with healthcare providers, community organizations, and provincial partners to strengthen referral pathways and expand program reach.
  • Maintain accurate client documentation and prepare regular program reports and outcome measures.
  • Contribute to quality improvement initiatives and support program evaluation.
  • Assist with planning and delivery of the annual New Brunswick Provincial Stroke Conference.
  • Represent Heart & Stroke NB professionally while promoting the organization's mission, vision, and values.

IDEAL QUALIFICATIONS

We're looking for someone who brings a combination of education, experience, and a genuine passion for helping others.

Preferred qualifications include:

  • Bilingualism (English/French) - with strong French Language skills
  • Experience working in healthcare, health promotion, community services, rehabilitation, or the non-profit sector.
  • Experience supporting clients through education, coaching, navigation, or case management.
  • Strong interpersonal, communication, and relationship-building skills.
  • Experience developing partnerships and engaging community stakeholders.
  • Excellent organizational skills with the ability to manage multiple priorities independently.
  • Strong problem-solving and critical-thinking abilities.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Valid driver's licence and willingness to travel throughout New Brunswick as required.
