Care Navigator -1 Full-Time (40 hours per week)

Qualifications:

Current and ongoing registration, or eligibility for registration, with the appropriate Nova Scotia Professional Regulatory body for one of the following healthcare professions: Nursing (LPN), Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.

Duties:

• Facilitate the admissions process in collaboration with the Continuing Care NS, family/resident, Clinical Lead, Neighborhood Manager, Office Manager and care team.
• Facilitate the review of admissions via the Pathways
• Support the transition to Long Term Care
• Educate the family on life and care in Long-Term Care
• Participating in Resident and Family Council
• Other duties as required
• May be required to work on a weekend to coordinate the placement process.
