Position Summary: 

The Team Aide is responsible to and works under the direction of the RN/LPN in providing care for the patient(s) which includes meeting their physical, social and psychological needs. Duties include, but are not limited to:

• Maintaining and assisting with the activities of daily living according to the patient’s capabilities.
• Recognizing the individuality of patients by involving them in decisions related to their care.
• Assisting in assessing, planning, implementing and evaluating patient care.
• Observing any unusual reactions, symptoms and incidents that may indicate changes in patient’s condition and reporting such to his/her supervisor.
• Recording appropriate information as determined by unit’s documentation system.
• Transporting patients as required.
• Collecting and distributing reports, specimens, slips and supplies to the appropriate locations.
• Understand and utilize appropriate aseptic techniques.
• Performing tasks as designated under the direction of the supervising nurse.
• Additional responsibilities for this position are to participate in activities related to the management and optimal functioning of the department on a daily basis.

Qualifications:

• Grade 12 education or equivalent required
• Continuing Care Assistant course required
• Previous experience in a hospital setting preferred
• Course in Alzheimer’s and related dementia required
